Objective:To analyze the correlation between clinical indexes of patients with blood stasis syndrome and early diabetic nephropathy and their correlation with renal function,to compare the risk factors related to diabetic nephropathy,and to lay the foundation for the research on the specific correlation between subsequent TCM syndrome types and western clinical indicators plus C hinese medicine provides more evidence for the prevention and treatment of diabetic nephropathy.Methods:From January 2019 to November 2019,90 DKD patients in the endocrinology department and inpatients of the Affiliated Hospital of C hengdu University of Traditional C hinese Medicine were selected as the research objects.All patients met the DKD diagnostic criteria.Refer to the 2016 "Practical Standards for Diagnosis of Blood Stasis Syndrome" to calculate the scores of TCM syndromes of DKD patients.The patients were grouped according to the TCM stasis syndrome scores.Patients with molecules within 0 to 2 points were used as the control group.Patients within the range of 2 to 12 points were classified into low groups.Those with more than 12 points and scores below 26 points Patients were assigned to the high group.The number of patients in the three groups was 28,32,and 30,respectively.Compare general clinical data(age,gender,duration)of patients in each group;compare total cholesterol,triglyceride,high-density lipoprotein,low-density lipoprotein,and index levels of each group;compare urinary albumin excretion rate,,microalbuminuria,?2 microglobulin,serum creatinine,urea nitrogen,blood uric acid,and glomerular filtration rate;compare systolic and diastolic blood pressure levels of patients in each group;compare hemoglobin levels.Results:Comparing the age,gender,and course data of patients in each group,the results showed that the course of disease in the high group was significantly different from that in the control group and the low group.The course of the high group was significantly higher,while the course of the low group was significantly higher than the control group.There were significant differences between the groups,P<0.05,indicating that the differences were statistically significant.After comparison,compared with the control group,patients in the low and high groups had ?2 microglobulin,urea nitrogen,urinary albumin excretion rate,Cystatin C,blood uric acid,and urine microalbumin/creatinine level was significantly higher,and the high group was significantly higher than the low group,while the glomerular filtration rate of patients in the high and low groups was significantly lower than that of the control group,and the level of glomerular filtration rate was lowest in the high group,P<0.05,the differences between groups were statistically significant.After comparison,compared with the control group,the patients in the low and high groups had significantly higher levels of total cholesterol,triglycerides,and low-density lipoprotein.The levels of the indicators above the high group were much higher than those in the low group.The levels of high-density lipoprotein,and hemoglobin were significantly lower than those in the control group.The high-density lipoprotein levels in the high-group patients were the lowest among the three groups,P<0.05.The differences between the groups were statistically significant.Compared with the control group,the systolic blood pressure and diastolic blood pressure of the patients in the low and high groups were significantly lower.The levels of the two indicators above the high group were much lower than those in the low group,P<0.05,and the differences between the groups were statistically significant..Spearman correlation analysis was used to analyze the correlation between various indicators and TCM symptom scores of blood stasis syndrome.The data showed that the indicators were positively correlated with TCM symptom scores of blood stasis syndrome including total cholesterol,triglycerides,low density lipoprotein,?2 microglobulin,urinary albumin excretion rate,C ystatin C,urinary microalbumin / creatinine,blood uric acid,urea nitrogen;negative correlation with TCM symptom score of stasis syndrome including glomerular filtration rate,high density lipoprotein,and hemoglobin.Multivariate regression analysis was performed on the statistically significant variables in the univariate analysis as independent variables,and the TCM syndrome score of the blood stasis syndrome as the dependent variable.The results showed total cholesterol,triglycerides,low density lipoprotein,and glomerular filtration rate,high-density lipoprotein,urinary albumin excretion rate,C ystatin C,urine microalbumin/creatinine,and urea nitrogen are all independent influencing factors of TCM syndrome scores of blood stasis syndrome.Conclusion:The blood stasis syndrome can affect the renal function indexes,lipid metabolism indexes,etc.of patients with diabetic nephropathy to a certain extent.The above indexes can be used as a reference for clinical TCM syndrome differentiation of patients with diabetic nephropathy.It can provide more theoretical basis for early diagnosis,early intervention and even preventing the occurrence,progression and deterioration of diabetic nephropathy and end-stage nephropathy.
