Background: Acute myocardial infarction(AMI)is a common malignant cardiac event in clinic.Ventricular remodeling after myocardial infarction is a common and serious complication of myocardial infarction.ECG is an important way to diagnose cardiac diseases.QT is the whole process of the ventricular depolarization and repolarization.QT dispersion is valuable in predicting myocardial remodeling and prognosis in patients with heart failure.The expression of micro RNA-125 b is up-regulated in myocardial tissue in patients with acute myocardial infarction and heart failure.Moreover,micro RNA-125 b plays a pivotal regulatory role in myocardial remodeling after infarction.Objective: To analyze the risk factors in patients with acute myocardial infarction and to analyze the correlation between QT dispersion and the expression of micro RNA-125 b in plasma.The prediction value of QT dispersion,plasma mi R-125 b level to ventricular remodeling after myocardial infarction was made by ROC curve.Methods: 132 patients with acute myocardial infarction diagnosed in our hospital from January 2015 to January 2017 were collected.QT dispersion and the expression of micro RNA-125 b of all the patients were tested and the outcome of ventricular remodeling was also followed up.According to the results above,the patients were divided into the observation group with ventricular remodeling after myocardial infarction and the control group without ventricular remodeling.The general data,such as gender,age,infarct location,type,risk factors,post-infarction complications,treatment methods and cardiac function were compared between the two groups.LVEDD,LVEDV,LVMI,LVPW,LVRI,LAP,CO,LVEF,QT dispersion and expression of micro RNA-125 b were also compared between the two groups.The risk factors of ventricular remodeling after myocardial infarction was analyzed by logistic regression,the correlation of QT dispersion,relative expression of micro RNA-125 b and ventricular remodeling was analyzed,and the diagnostic value of combination of micro RNA-125 b and QT dispersion in ventricular remodeling was determined eventually.Results:1.Comparison of general data between the two groups: 52 of 132 patients had ventricular remodeling.The incidence rate was 39.39%.Hardy-Weinberg equilibrium test showed that the loci conformed to Hardy-Weinberg equilibrium,indicating that the data were reliable and representative.There was no significant difference in age,gender,smoking history,drinking history,pathological location,BMI,HDL,pathological vessel diameter between the two groups(P>0.05).There was significant difference in hypertension,diabetes,cholesterol,triglyceride,LDL,cardiac function,lesion length,hs CRP between the two groups(P<0.05).2.Comparisons of color Doppler indexes between the two groups:LVEDD,LVEDV,LVMI and LVPW in the observation group were significantly higher than those in the control group(P < 0.01),while LVRI,LAP,CO and LVEF in the observation group were significantly lower than those in the control group(P < 0.01).3.Comparison of QT dispersion between the two groups:QTd and QTcd in the observation group were(78.82±14.93)ms and(62.39±8.76)ms,respectively,which were significantly higher than those in the control group(88.81±20.21)ms and(73.19±20.33)ms(P < 0.01).4.Comparison of the relative expression of micro RNA-125 b between the two groups: the relative expression of micro RNA-125 b in the observation group was(1.394±0.494)and in the control group was(0.974±0.423).It was significantly higher in the observation group than that in the control group(P < 0.01).5.The analysis of the risk factors of ventricular remodeling:Based on the comparison of one-way ANOVA between the two groups,the indexes of hypertension,diabetes,cholesterol,triglyceride,LDL,cardiac function,lesion length,hs CRP,mi R-125 b and QTd were included in logistic regression analysis.Results: diabetes,hypertension,hs CRP,cardiac function,lesion length,mi R-125 b and QTd were independent risk factors of ventricular remodeling after myocardial infarction Risk factors.6.Correlation analysis between relative expression of micro RNA-125 b and LVEDV:Regression showed that LVEDV was positively correlated with the expression of micro RNA-125 b,taking LVEDV as dependent variable and micro RNA-125 b as independent variable,Y=37.77 X + 43.51(r=0.561,P=0.000).7.Analysis of the correlation between QT dispersion and LVEDV:Regression analysis showed that LVEDV was positively correlated with QT dispersion range,taking LVEDV as dependent variable and QT dispersion as independent variable,Y=1.718X-28.77(r=0.593,P=0.000).8.Correlation analysis between QT dispersion and relative expression of micro RNA-125b:Regression showed that QT dispersion was positively correlated with micro RNA-125 b,taking QT dispersion as dependent variable and micro RNA-125 b as independent variable,Y=12.44X+46.70(r=0.516,P=0.000).9.The diagnostic value of QT dispersion and relative expression of mi R-125 b for ventricular remodeling: ROC curve showed that the area under the curve of QT dispersion for diagnosis of ventricular remodeling was 0.889,the area under the curve of 95% CI was 0.833-0.945,micro RNA-125 b for diagnosis of ventricular remodeling was 0.812,and 95% CI was 0.738-0.886.Conclusions:1.Diabetes mellitus,hypertension,hs CRP,cardiac function and lesion length are the traditional risk factors of ventricular remodeling after myocardial infarction.2.QT dispersion and mi R-125 b are also risk factors of ventricular remodeling after myocardial infarction,and have a better predictive value for the occurrence of ventricular remodeling in patients with myocardial infarction.
