Clinical Analysis Of 104 Cases Of Stanford Type B Aortic Dissection

Objective: To review the clinical characteristics and treatment methods of Stanford type B aortic dissection patients admitted in our hospital in the last two years,and summarize the clinical experience to improve the level of diagnosis and treatment.Methods: A total of 104 Stanford type B aortic dissection patients were admitted to the Fourth Hospital of Hebei Medical University from January 2018 to December 2019.Collect each patient's name,gender,age,first symptoms,onset time,past medical history,vascular imaging diagnostic data(Computed Tomography angiography),surgical method,anesthesia method,related complications,hospitalization days.Finally,The clinical data was analyzed statistically.Results: 104 patients,all have surgical indications,of which 91 patients chose surgical treatment,all stent-graft are successfully placed,the technical success rate of 100%,after surgery reverse tear A type aortic dissection death 2 cases,5 cases of small internal leakage,3 cases of renal insufficiency,4 cases of gastrointestinal bleeding,4 cases of delirium,in addition,1 case each in patients with tracheoesophagifist fistula,stent displacement,laryngeal edema and Cerebral Infarction.Thirteen patients chose conservative treatment,2 deaths during hospitalization and 3 deaths after discharge,all of which were considered for aortic rupture.A total of 7 patients died in the whole group,and the rest were not specific at present.Conclusion: 1,The aortic dissection is good in middle-aged,and males are more common,clinical symptoms are complex,avoiding misdiagnosis in clinical work.2,Thoracic endovascular aortic repair with its little trauma,fast recovery,and excellent clinical results and so on,has become the preferred treatment method of The Stanford type B aortic dissection,coupled with the progress of surgical technology and the development of materials,the application of TEVAR will be more widely in the future.3,local infiltration anesthesia has the advantages of rapid recovery,less complications,low cost,etc.,some patients had a good performance status and no serious underlying diseases,local infiltration anesthesia is safe and feasible.
