OBJECTIVE: To compare the efficacy of internal fixation bone plate and titanium alloy bone plate made of antibiotic composite bone cement material in rabbit tibia fractures with infection,and to explore the feasibility of its application.Methods:(1)60 New Zealand white rabbits were randomly divided into a control group(titanium alloy bone plate group)and an experimental group(antibiotic bone cement plate group).(2)The two groups of experimental subjects were made into tibia by wire sawing.The fracture of the middle section was inoculated with Staphylococcus aureus to make a model of fracture and infection.The control group used titanium alloy bone plate for fracture internal fixation.The experimental group was treated with antibiotic bone cement to form a bone plate that completely fits the fracture site.(3)Both groups were given a broad-spectrum antibiotic(gentamicin sulfate)throughout the body after surgery.The general conditions of the subjects were observed daily and recorded.The two groups of subjects underwent X-ray examinations at 2,4,and 8 weeks after surgery,and the results of each X-ray were evaluated according to the X-ray film epiphyseal evaluation criteria developed by Johnson et al.To observe the healing.(4)On the 1st,3rd,7th,and 14 th days after the operation,blood samples were collected for blood routine and C-reactive protein detection.At 2 weeks after operation,histopathology and surrounding soft tissues were examined by histopathology after the internal fixation was removed,and the healing of the fractures and the control of the surrounding soft tissue infection were observed in both groups.Results: 1.Laboratory examination results: The white blood cell counts in the venous blood of the white rabbits on the first and third days after operation showed that there was little difference between the experimental group and the control group(P> 0.05).Counting results showed that the experimental group was significantly lower than thecontrol group(P <0.05).The CRP content measurement results showed no significant difference between the experimental group and the control group on the first day(P>0.05),but the CRP content of the experimental group was significantly lower than the control group on the 3rd,7th,and 14 th days,and the difference was statistically significant(P <0.05)Especially on the 7th day,the CRP content in the experimental group was significantly lower than that in the control group(P <0.01).2.X-ray examination results: Compare the infected and uninfected X-ray examination results of the experimental group with the control group at 2 weeks after operation.In the X-ray results of the control group and the experimental group at 2 weeks after operation,the infected rabbit model experimental group No significant difference was found between the control group and the uninfected rabbit model.3.Histopathological observation results: The pathological examination results of the fracture sites in each group at 2 weeks after operation showed that the number of chondrocytes and osteoblasts in the two groups was similar in the same healing period,and the degree of ossification of the stump chondrocytes was similar.The number of S.aureus colonies in the soft tissue around the experimental group was significantly lower than that in the control group.Conclusions:(1)There is no significant difference in the healing effect of titanium alloy bone plate and antibiotic bone cement plate in the treatment of rabbit tibia fracture internal fixation;(2)antibiotic bone cement plate for local growth of Staphylococcus aureus.The inhibition strength is obviously higher than that of titanium alloy bone plate.
