Abstract/Summary:
Objective:Although artificial liver support system may improve the therapeutic response rate and serve as a bridge to liver transplantation for liver failure of various causes, clinical data on its efficacy in the treatment of alcoholic liver failure are lacking. To this end,we conducted this retrospective study to preliminarily evaluate the tolerance and efficacy of artificial liver support system in treating patients with alcoholic liver failure and the prognostic factors were further analyzed. Methods:Patients with alcoholic liver failure who received artificial liver treatment in our hospital from January 2015 to January 2019 were selected.General clinical data and biochemical data of the patients before and after the artificial liver treatment were collected, and the effect observation node was selected for 12 weeks to statistically analyze the treatment effectiveness.Furthermore, the indicators associated with the prognosis were identified. Results:All patients had jaundice as the major symptom, and most of the patients also combined with fatigue, gastrointestinal symptoms. After receiving the artificial liver treatment, the clinical symptoms of the patients were significantly relieved, including the grade of hepatic encephalopathy decreased or the symptoms disappeared in some patients. After the first artificial liver treatment,ALT,ALP and GGT decreased significantly(P<0.05),while AST,TB and TBA improved significantly(P<0.001).After the last artificial liver treatment,AST,ALP,PT and INR decreased significantly(P<0.05),while TB and TBA improved significantly(P<0.001).A mild rash appeared in3 patients and there were no serious adverse reactions such as hypotension and imbalance syndrome. The overall effective rate of treatment was 64.52%.Univariate analysis showed statistical difference in age, level of alcohol consumption, drinking years, infections, electrolyte disturbances, hepatorenal syndrome,TB,INR,and PT between patients with different prognosis in the two groups(P <0.05),suggesting that it may be associated with poor prognosis of alcoholic liver failure treated by artificial liver.Conclusions:1.Artificial liver treatment was safe and well tolerated in patients with alcoholic liver failure. After treatment, the clinical manifestations and biochemical indicators of the patients are improved significantly. The overall effective rate of treatment is64.52%.2.Age,level of alcohol consumption, drinking years, infection, electrolyte disturbance, hepatorenal syndrome,TB,INR,and PT may be associated with poor prognosis of alcoholic liver failure treated by artificial liver.
