Objectives This issue is a comprehensive retrospective analysis of HBV infection and Alcoholic liver disease(ALD)patients' general data,laboratory examination and other indicators.To further explore the risk factors of liver failure in patients with HBV infection complicated with alcoholic liver disease.Methods Retrospective analysis of December 2007-June 2017 in North China University Hospital and Tangshan Infectious Disease Hospital inpatients with HBV infection with alcoholic liver disease in 104 patients with liver failure as a case group.Select the same period without concurrent liver failure in 312 patients as a control group.Analysis of all patients in the age,gender,duration and other general baseline data;Blood routine: white blood cell(WBC),red blood cell(RBC),hemoglobin(Hb),platelet(PLT).Liver function: alanine aminotransferase(ALT),aspartate aminotransferase(AST),albumin(ALB),cholinesterase(CHE),total bilirubin(TBIL),and CTP score.Renal function: creatinine(Cr),urea nitrogen(BUN).Blood sugar(GLU);blood lipid: cholesterol(CHO),triglyceride(TG);electrolyte: potassium ion(K),sodium ion(Na).Blood coagulation,fibrinogen(FIB),prothrombin time(PT),thrombin time(TT),activated partial thromboplastin time(APTT).Complications: upper gastrointestinal bleeding,infection,ascites,hepatic encephalopathy,hepatorenal syndrome and so on.All data were statistically analyzed by SPSS19.0 statistical software.Independent samples t test was used to measure data.Mean square deviation was expressed by mean square deviation.Chi square test was used to count data,and P < 0.05 indicates difference is statistically significant.The risk factors of HBV infection combined with alcoholic liver disease complicated with liver failure were selected by Logistic regression analysis.Results 1 The case group and the control group comparison in sex,Age,Hypertension,Diabetes,Coronary heart disease,Blood type,Cholelithiasis,WBC,RBC,Hb,PLT,Cr,GLU,CHO,TG,K,PT,APTT,P>0.05,the difference is not statistically significant.2 The case group and the control group comparison in family history(?~2=4.411,P=0.036);Child-Pugh classification(?~2=18.227,P=0.000);ascites(?~2=20.399,P=0.000);hepatic encephalopathy(?~2=14.557,P=0.001);Upper gastrointestinal bleeding(?~2=8.718,P=0.003);Hepatorenal syndrome(?~2=6.615,P=0.010);infection(?~2=4.649,P=0.031);ALT(199.30±36.38 vs 141.37±29.88,t=3.014,P=0.013);AST(215.17±41.40 vs 162.33±29.86,t=2.535,P=0.030);ALB(28.56±6.55 vs 34.78±6.82,t=3.683,P=0.000);CHE(3569.91±1845.04 vs 4320.83±2333.02,t=2.954,P=0.003);TBIL(215.24±52.51 vs 88.45±43.18,t=15.826,P=0.000);CPT score(13.43±1.41 vs 10.76±1.07,t=3.260,P=0.012);BUN(19.93±6.81 vs 11.43±5.49,t=2.421,P=0.036);Na(127.04±14.31 vs 138.30±13.33,t=4.008,P=0.000);FIB(2.81±1.32 vs 2.45±1.01,t=2.121,P=0.035);TT(12.16±3.39 vs 13.87±4.97,t=3.251,P=0.001)and other aspects,P <0.05,the difference is statistically significant.3.Multivariate logistic regression analysis showed: CTP score,upper gastrointestinal bleeding,ascites and total bilirubin are risk factors for HBV infection complicated with alcoholic liver disease complicated with liver failure.Conclusions The higher the CPT score,the upper gastrointestinal bleeding,the ascites and the higher the total bilirubin are the risk factors for the HBV infection complicated with alcoholic liver disease complicated with liver failure.
