Background: upper blepharoplasty is one of the most popular aesthetic surgery in the world.Traditional approach of incision cannot avoid Linear scars which could be seen obviously when eyes closed.This study set out to explore a secluded incision for upper blepharoplasty.Methods: A total of 300 patients underwent upper blepharoplasty surgery from July 2017 to December 2018.180 of them underwent traditional external incision(TEI)and others underwent palpebral margin incision(PMI).PMI group includes 5 cases with with excess upper eyelid skin,13 cases of single-eyelid with mild to moderate ptosis treated with blepharoplasty surgery with ptosis correction through palpebral margin incision(PMI).All patients were followed up for 6 to 12 months with an average of 8 months.The outcomes were evaluated based on edema,scar visibility,complications and patient satisfaction,and the differences between the two groups were compared.Results: The satisfaction score of the PMI group was 95.03±3.02,which was significantly higher than that of the control group(89.18±4.04),and the incidence of postoperative swelling and scarring was lower in the observation group,the differences were statistically significant(P<0.05).Complications of the TEI group mainly included scar(5.5%),prolonged edema(3%)and unsatisfactory double eyelid Line morphology(4%).Patients in the PMI group showed no dissatisfaction with scar and edema time(0%),and other postoperative complications occurred in 6 patients(2%).One patient experienced the disappearance of the crease Line.One case had a knot exposed one month after surgery,and a dermoid cyst occurred two months after surgery.Four patients developed exposure single suture knots at 1 to 2 months after surgery and achieved satisfactory results after removing the knot.Conclusions: This PMI technique is a safe and effective surgical method for upper blepharoplasty with many advantages such as no visible scar,shorter recovery time and less edema etc.This method may suitable for some kinds of patients and deserved to be promoted in clinical.
