Objective:1.To observe the clinical efficacy of Shenling Baizhu Powder in treating spleen deficiency and dampness excess type Irritable Bowel Syndrome with Predominant Diarrhea(IBS-D).2.To observe the changes of serum Diamine Oxidase(DAO)and D-lactate(D-LA)contents of intestinal barrier function indexes in patients with spleen deficiency and dampness excess type IBS-D treated with Shenling Baizhu Powder,and to explore the relationship between intestinal barrier function and IBS-D.3.To explore the possible modern medical mechanism of Shenling Baizhu Powder in treating spleen deficiency and dampness excess type IBS-D.Methods:1.Seventy-two spleen deficiency and dampness type IBS-D patients who visited outpatient department of digestive endoscopy of our hospital from February 2019 to January2020 and met the inclusion criteria were selected as the study subjects.According to random number table method,they were randomly divided into the Chinese medicine treatment group(n=36)and the western medicine control group(n=36).Meanwhile,36 healthy people from the physical examination center of our hospital were selected as healthy control group.2.The Chinese medicine treatment group was given Shenling Baizhu Powder,and the western medicine control group was given Pinaverium Bromide tablets.The course of treatment was 4 weeks.In this study,a total of 4 patients in the Chinese medicine treatment group and the western medicine control group did not complete the test,and the results were not included in the study results.3.Collected the clinical data of the patient’s age,gender,course of disease,et al.The Chinese medicine symptoms score was observed and recorded before and after treatment.The ELISA method was used to detect serum DAO and D-LA contents during the time.Results:1.Comparison of clinical data: Comparing the gender composition and age of the three groups of subjects,the difference was not statistically significant(P>0.05)and was comparable.There was no statistically significant difference in course of disease between the Chinese medicine treatment group and the western medicine control group(P>0.05),suggesting comparability.2.Comparison of single symptom scores: Before treatment,there was no statistically significant difference in all single symptom scores between the Chinese medicine treatment group and the western medicine control group(P>0.05),suggesting comparability.After treatment,the scores of each single symptom from Chinese medicine treatment group significantly decreased(P<0.01),which overmatched the western medicine control group(P<0.05)except for abdominal pain.And the advantages in fecal frequency,fecal characteristics,mental fatigue and burnout were especially obvious(P<0.01).3.Comparison of total Chinese medicine symptom scores: Before treatment,there was no statistically significant difference in the total Chinese medicine symptom scores between the Chinese medicine treatment group and the western medicine control group(P>0.05),suggesting comparability.After treatment,the total Chinese medicine symptom scores in both groups were significantly improved compared with that before treatment(P<0.01),and theimprovement degree in the Chinese medicine treatment group was better than that in the western medicine control group(P<0.05).4.Comparison of efficacy of Chinese medicine symptom: The total effective rate of the Chinese medicine treatment group was 85.29%,and that of the western medicine control group was 73.53%,the former was better than the latter in the efficacy(P<0.05).5.Comparison of serum DAO and D-LA contents: The serum DAO and D-LA contents in the Chinese medicine treatment group and the western medicine control group were significantly higher than those in the healthy control group before treatment(P<0.01).Before treatment,there was no statistically significant difference between the two groups of patients(P>0.05),which was comparable.After treatment,the serum DAO and D-LA contents in the two groups of patients were significantly lower than those before treatment(P<0.01),and the Chinese medicine treatment group decreased more than the western medicine control group(P<0.05).Conclusion:1.Shenling Baizhu Powder can effectively improve the symptoms of spleen deficiency and dampness type IBS-D patients,without obvious adverse reactions.2.Serum DAO and D-LA contents in patients with spleen deficiency and dampness excess type IBS-D are higher than those in healthy patients and decreased after treatment,indicating that intestinal barrier dysfunction may be one of the pathogenesis of IBS-D.3.The improvement of intestinal barrier function may be one of the mechanisms of Shenling Baizhu Powder in treating spleen deficiency and dampness excess type IBS-D.
