| Objectives(1)To investigate the frailty degree of the community-dwelling older of Lanzhou city,it can provide a reference for the comparison of the incidence of debilitating between regions.(2)To explore the influencing factors in depth,and provide theoretical basis for delaying the frailty the community-dwelling older adults and realizing the healthy aging.Methods(1)Four hundred fifty nine older ≥65 age years old were selected from two community hospitals in Chengguan district,Lanzhou city,Gansu province from July 2017 to July 2018.Frailty status was assessed by The Fried phenotype and the older adults were divided into three groups: non-frailty,pre-frailty and frailty.The self-made general data questionnaire,the Mini-Mental State Examination(MMSE),Instrumental Activity of Daily Living(IADL),and Mini Nutritional Assessment(MNA)and Self-rating Depression Scale(SDS)explore the influencing factors of the frailty among the community-dwelling older adults.(2)Epidata3.1 software was be used to establish database input data,and SPSS 22.0 software was be used to analyze the data.Measurement data are expressed as((?)±s)and comparisons between measurement data groups that meet the normal distribution are analyzed by analysis of variance.Non-normally distributed measurement data uses non-parametric tests;count data is expressed as(n,%),and comparison between groups uses chi-square test or Fishers exact test.The influencing factors of weakness were analyzed by multiple logistic regression.The above test standards are statistically significant with P <0.05.Results(1)The incidence of frailty was 10.1%.Among the 459 the community-dwelling older adults,46 were frailty(10.1%);282 pre-frailty(61.4%);and 131 non-frailty(28.5%).The five items of frailty are in descending order: 176 cases with low grip strength(38.3%),147 cases with slow gait speed(32.0%),125 cases of fatigue(27.2%),69 cases of weight loss(15.0%),and 12 cases of low physical activity(2.6%).(2)Univariate analysis showed that there were statistically significant differences in age,education,nutritional status,depression,and cognitive function among the three groups of non-frailty,pre-frailty,and frailty older adults(P <0.05);(3)Multiple logistic regression analysis showed that age(OR: 1.210,95% CI: 1.116 ~ 1.313),SDS score(OR: 1.125,95% CI: 1.041 ~ 1.216),BMI≥ 28.0kg / m2(OR: 2.691,95% CI: 1.061 ~ 6.825),≥ 4 kinds of diseases(OR: 2.612,95% CI: 1.331 ~ 5.124)are the risk factors for the degeneration of the elderly in the community(OR> 1,P <0.05);and MNA score(OR: 0.669,95% CI: 0.547 ~ 0.819),MMSE score(OR: 0.767,95% CI: 0.667 ~ 0.882)are the protective factors for the degeneration of the elderly in the community(OR <1,P <0.05).Conclusion(1)The incidence of frailty in the community-dwelling older adults of Lanzhou city was higher,which should be taken seriously by community nursing staff.(2)The older adults who are older,low education,poor nutritional status,IADL dysfunction,cognitive decline and depression are more prone to frailty.Community care workers should pay more attention to these groups.(3)It is suggested to include frailty screening into the routine physical examination program for the older adults in the community,so improved decision making and possibly early intervention,therefore preventing cognitive impairment occurs. |
