ObjectiveTo observe the proportion of CD4~+T cell and Tregs and the expression of IL-35 in peripheral blood of patients with acute herpes zoster,and to further explore the relationship between the expression of Treg cells and IL-35 in acute patients and the development of postherpetic neuralgia.Material and methodsFrom November 2017 to October 2018,68 patients with acute herpes zoster who visited the department of dermatology of our hospital were recruited,and other 20healthy subjects were selected as the control group.Peripheral venous blood was collected on the day of their first visit.The proportion of CD4~+T cell and CD4~+CD25~+FoxP3~+Treg in peripheral blood was detected by flow cytometry.Total RNA was extracted from peripheral blood PBMC by Trizol method,and the expression of IL-35(EBI3 and p35)mRNA in PBMC was analyzed after fluorescence quantitative reverse transcription PCR amplification.Serum levels of cytokine IL-35 were detected by ELISA.Patients were followed up three months after the initial visit,and then were divided into PHN group and No-PHN group.Results1.Among the 68 patients,21(30.88%)developed postherpetic neuralgia.The mean age was significantly older in PHN group than in No-PHN group.The degree of acute pain was more severe in PHN group than in No-PHN group.2.The proportion of CD4~+T cells in peripheral blood of patients with acute herpes zoster was significantly lower than that of the control group(t=5.440,P<0.001).The proportion of CD4~+T cells in PHN group was lower than that in No-PHN group(t=2.300,P<0.05).3.The proportion of Tregs in peripheral blood of patients with acute herpes zoster was significantly higher than that of the control group(t=6.656,P<0.001).The proportion of Tregs in PHN group was higher than that in No-PHN group(t=3.856,P<0.001).4.The expression of EBI3mRNA in the acute herpes zoster group was significantly lower than that of the control group(t=2.521,P<0.05),There was no significant difference in P35mRNA between acute herpes zoster group and control group(t=1.725,P=0.096).There was no significant difference in IL-35(EBI3 and P35)mRNA expression between PHN group and No-PHN group(P>0.05).5.Compared with the control group,serum IL-35 level in patients with acute herpes zoster decreased significantly(t=2.820,P<0.01),and there was no statistically difference between PHN group and No-PHN group(t=1.016,P>0.05).Conclusion:1.proportion of CD4+T lymphocytes in peripheral blood of patients with acute herpes zoster decreased,with the decreasing proportion of Treg cells,acute pain in patients are more prone to becoming chronic,indicating that the suppression of cellular immune function plays an important role in the process of AHZ and PHN.2.The proportion of Treg cells in peripheral blood of patients with acute herpes zoster increased.With the increasing proportion of Treg cells,acute pain in patients are more prone to becoming chronic.Moreover CD4+T cells were inversely correlated with the change of Treg cells.These results suggest that Treg cells may play an important role in negative regulation of T lymphocyte subsets in herpes zoster.3.In herpes zoster,IL-35 may not participate in the cellular immunosuppressive function of Treg cells,or participate in the currently unknown mechanism,which needs to be illuninated by further studies in the future.
