| Research background and purpose The incidence and mortality of breast cancer are increasing year by year,and it has become one of the most common malignant tumors in women all over the world and the age of onset gradually tends to be younger.With the progress of the society,the improvement of living conditions and the improvement of health awareness,the attention of mammary gland disease is also increasing.So far,there have been many methods of screening and diagnosis of breast diseases.The most commonly used imaging methods in clinical practice mainly include full-screen digital mammography(or whole-field digital mammography)(Full-field digital mamography,FFDM),ultrasonography and MRI.However,these conventional imaging methods have their own shortcomings,for example,there are limitations in sensitivity or specificity.A new inspection technique needs to be improved and improved.In recent years,digital photography technology has been developing and updating,and new technologies are emerging,of which DBT is one of the new mammography techniques.After many foreign scholarsâ prospective research on this technology,it is proved that DBT has unique advantages in the detection and diagnostic accuracy of breast diseases.However,the technology has not been widely used in China.In all types of diseases of the breast,mass is not only the most common clinical first symptom,but also the most basic imaging signs of breast disease.Both benign and malignant lesions can exist as masses.The purpose of this study was to compare the diagnostic efficacy of digital mammography and conventional imaging in breast mass.Data and methods 71 cases of breast lump-type lesions were selected and all patients had complete imaging and pathological data.All patients had received FFDM,DBT,b-mode ultrasound and MRI before operation.Retrospective analysis was conducted on the images obtained by the four examination methods,according to the breast imaging report and data system(bi-rads)of 2013,referring to the final postoperative pathological results.The diagnostic performance of the above four inspection methods was compared and analyzed using the Receiver operating characteristic(ROC)curve,and the statistical differences between DBT and other three methods were compared by Z test.The Average Glandular Dose(AGD)of FFDM and DBT in the same patient under the same compression condition was recorded,t test was used to compare the statistical differences between the two methods.Result All the 71 patients were single breast lesions,and a total of 71 lesions were found,including 44 benign lesions and 27 malignant lesions.Area under the curve(AUC)of FFDM,DBT,sonography,and MRI based on the BI-RADS were0.723,0.869,0.697 and0.818.The paired comparison results showed that the AUC of DBT was higher than that of FFDM and b-mode ultrasound,and the difference was statistically significant(P<0.05).DBT has higher specificity and accuracy than MRI.(P>0.05),and the difference was statistically significant(P<0.05).The AGD of DBT was higher than FFDM,and the difference was statistically significant(P<0.05).However,the AGD for both methods was significantly lower than the maximum single dose of radiation(3m Gy)set by the radiography quality standards act of American society(MQSA).Conclusion Compared with FFDM and b-ultrasonography,DBT can significantly improve the diagnostic efficiency of benign and malignant breast masses;compared with MRI,the diagnostic efficacy is similar,but the specificity and accuracy are higher than MRI.As a simple,safe and reliable new mammography technique,DBT has a good application prospect. |
