Objective:The effects of sleep disordered breathing evaluated by sleep structure and sleep respiration hypoventilation index on the degree of chronic heart failure were analyzed.Methods:Sixty-three patients who underwent sleep respiration monitoring from January 2017 to January 2020 in the Second Hospital of Shanxi Medical University were collected and divided into a heart failure group and a non-heart failure group.Based on the sleep respiratory monitoring results,the heart failure group are divided into sleep-disordered breathing group and non-sleep-disordered breathing group.NT-proBNP and LVEF were measured in three groups:non-heart failure group,sleep-disordered breathing group,and non-sleep-disordered breathing group.AHI,nocturnal sleep structure(Nl1 N2,N3-N4,REM,awakening period)and nocturnal blood oxygen saturation were statistically analyzed.Result:The 63 patients collected had no significant differences in age,gender,BMI,smoking history(P>0.05).There are differences in drinking history,history of hypertension and coronary heart disease(P<0.05).The patients were divided into non-heart failure group(n=38)and heart failure group(N=25).The incidence of sleep-disordered breathing(SDB)in non-heart failure patients was different from that in patients with heart failure(P=0.011);patients with heart failure combined with SDB NT-proBNP was higher(P=0.016);left ventricular ejection fraction was reduced in patients with heart failure after sleep apnea(P=0.008);an the NYHA III group,the N1-N2 phase was longer than that of the NYHA ? group(P=0.005),and the REM in the NYHA ? group was shorter than that of the NYHA ? group(P=0.003).However,the N3+N4 phase showed consistent results(P=0.656);the average blood oxygen saturation and the minimum blood oxygen saturation of different degrees of heart failure were different(P=0.01,P,0.012).Correlation analysis of NYHA grading and AHM,awakening N1N2,N3+N4,R phase,the results suggest that AHI,awakening N1N2 and NYHA grading all show significance,and the correlation coefficient value is greater than 0,which means AHI,awakening N1N2 and NYHA grading has a positive correlation(P<0.05).The R phase and the NYHA classification all showed significance,and the correlation coefficient values were-0.548,which means a negative correlation(P<0.05).There is no correlation between N3+N4 and NYHA classification(P>0.05).Conclusion:Among the patients who have completed sleep breathing monitoring clinically,most patients with chronic heart failure have SDB,and the incidence is higher than that of non-heart failure patients.When patients with chronic heart failure combined with SDB,the average blood oxygen saturation and the lowest blood oxygen saturation during sleep decreased,light sleep increased and REM sleep decreased,and the length of light sleep was positively correlated with the degree of heart failure;REM phase sleep was negatively correlated with the degree of heart failure;compared with patients with heart failure alone,heart failure patients with SDB had lower LVEF,NT-proBNP is significantly higher.
