Objective:To explore the diagnostic significance of red blood cell distribution width(RDW)and neutrophil lymphocyte ratio(NLR)in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)complicated with pulmonary thromboembolism(PTE).Methods:A total of 40 patients with AECOPD and PTE admitted to the Department of Respiratory and Critical Care Medicine of Shanxi Bethune hospital from January 2017 to December 2018 were enrolled.60 patients with AECOPD were included in the non-combined group.Fifty patients were in the control group.The RDW,NLR,D-dimer,white blood cell count(WBC),platelet count(PLT),platelet distribution width(PDW),and platelet mean volume(MPV)of each group were retrospectively analyzed.Oxygen partial pressure(PO2),partial pressure of carbon dioxide(PCO2),blood pressure(BP),body mass index(BMI),using receiver operating characteristic curve(ROC)to evaluate the diagnosis of AECOPD combined with PTE by RDW,NLR and their combined detection value.Results:(1)The RDW,NLR,D-dimer and WBC of the combined group and the non-combined group were higher than those of the control group.The RDW,NLR and D-dimer of the combined group were also higher than the non-combined group,and there was no significant difference in WBC.The PO2 of the combined group and the non-consolidated group was lower than that of the control group,and the PO2 of the combined group was also lower than that of the non-combined group.The PCO2 of the non-combined group was higher than that of the combined group and the control group,and there was no significant difference in PCO2 between the combined group and the control group.(2)There were no significant differences in PLT,PDW,MPV,BP,and BMI between the three groups.(3)The ROC curve predicted AECOPD combined with PTE.The area under the RDW curve was AUC=0.580(sensitivity 85%,specificity 41.7%),and the area under the NLR curve was AUC=0.560(sensitivity 82.5%,specificity 33.3%).The area under the curve is AUC=0.610(sensitivity 90%,specificity 40%).Conclusion:RDW and NLR are of great significance for diagnosing AECOPD with PTE.The combined detection of the two can be used as a diagnostic indicator for AECOPD combined with PTE.
