| Objective:To investigate the effects of mindfulness-based cancer rehabilitation on cancer-induced fatigue in patients undergoing chemotherapy after lung cancer surgery.Methods:From December 2018 to December 2019,researchers divided 120 lung cancer patients who met the inclusion and exclusion criteria for thoracic surgery in a general hospital into an intervention group(60 cases)and a control group(60 cases)by flipping a coin.All patients in both groups received routine education and nursing care,the control group received routine guidance,and the intervention group received 8 weeks of mindfulness cancer rehabilitation training.One training topic per week.And at least 6 times a week to carry out mindful cancer rehabilitation training,each lasting about 30 minutes.The researchers used General Demographic Questionnaire,Cancer Fatigue Scale(CFS),Self-rating Anxiety Scale(SAS),Self-rating Depression Scale(SDS),General self-efficacy Scale(GSES)and the Mindfulness Attention Awareness Scale(MAAS)measured the levels of cancer-related fatigue,mindfulness,general self-efficacy,anxiety and depression in the two groups before,after,and at the first and third months after intervention,respectively.In addition,the researchers also measured mindfulness levels in all the patients after their fourth week of practice.SPSS22.0statistical software was used to analyze the data.The scores of cancer-induced fatigue,anxiety,depression,general self-efficacy,and mindfulness level were compared between the two groups using the repeated measures anova.The paired t test was used to compare the levels of cancer-induced fatigue,anxiety,depression,general self-efficacy and mindfulness in the two groups before and after intervention.Two independent sample t test was used to compare the levels of cancer-induced fatigue,anxiety,depression,general self-efficacy and mindfulness between the two groups after intervention.Non-parametric tests were used to compare the levels of cancer-related fatigue,anxiety,depression,general self-efficacy and mindfulness between the two groups at different time points.When P<0.05,the difference was statistically significant.Results:1.41 cases were lost to follow-up.There were 40 cases in the intervention group and 39cases in the control group.By comparing the number of 41 lost follow-up cases with the general demographic data of 79 cases,the results showed that the difference between the lost follow-up group and the non-lost follow-up group was not statistically significant(P>0.05).2.Baseline data results showed that the scores of general demographic data,cancer-induced fatigue,general self-efficacy,anxiety,depression,and mindfulness before intervention were comparable between the two groups,with no statistically significant differences(P>0.05).3.After intervention,the total fatigue and each dimension of fatigue,anxiety and depression scores in the intervention group at different times were lower than those in the control group.After intervention,the scores of self-efficacy and mindfulness level of the intervention group were higher than those of the control group.4.The results of repeated measures anova showed that the total fatigue,fatigue in all dimensions,general self-efficacy,mindfulness level,anxiety and depression scores of the intervention group and the control group were statistically significant at different time points,under different interactions and under different treatments(P<0.05).According to the summary chart of repeated measurements,with the change of time,the decrease of total fatigue and fatigue in each dimension,anxiety and depression scores in the intervention group was greater than that in the control group,and the increase of general self-efficacy and mindfulness level scores in the intervention group was greater than that in the control group.5.Comparison between groups:before the intervention,there was no statistically significant difference between the two groups in total fatigue and scores of each dimension,general self-efficacy,mindfulness level,anxiety and depression(P>0.05).There were statistically significant differences in total fatigue and other dimensions,general self-efficacy,mindfulness level,and anxiety scores between the two groups after 8 weeks of intervention,1month and 3 months of follow-up(P<0.05).After 4 weeks of intervention,there was a statistically significant difference in mindfulness level scores between the two groups(P<0.05).After 8 weeks of intervention,there was no statistically significant difference in depression scores between the two groups(P>0.05).After 1 month and 3 months of follow-up,there were statistically significant differences in depression scores between the two groups(P<0.05).Intra-group comparison:at different time points,the total fatigue and each dimension,general self-efficacy,mindfulness level,anxiety and depression scores of the intervention group were statistically significant(P<0.05).6.Pairwise comparison at different times:With the exception of T2 and T4(P>0.05),pairwise comparison of total fatigue scores in the intervention group at other times showed statistically significant differences(P<0.05).With the exception of T2,T4,T3 and T4(P>0.05),the physical fatigue scores of patients in the intervention group were statistically significant when pairwise compared at other times(P<0.05).Except T1,T2,T3 and T4(P<0.05),there was no statistically significant difference in the emotional fatigue scores of the intervention group when pairings were conducted at other times(P>0.05).Except for T2 and T4(P>0.05),there were statistically significant differences in the cognitive fatigue scores of patients in the intervention group when compared in pairs at other time points(P<0.05).When the general self-efficacy scores of the intervention group were compared in pairs at different times,the differences were statistically significant(P<0.05).In addition to T3 and T4(P>0.05),there were statistically significant differences in the level of mindfulness scores of the intervention group when compared in pairs at other time points(P<0.05).Except for T1,T2,T3 and T4(P<0.05),pairwise comparison of patients in the intervention group at other time points showed no statistically significant difference(P>0.05).Except T3 and T4(P>0.05),the anxiety scores in the intervention group was statistically significant when pairwise comparison was conducted at other time points(P<0.05).Conclusion:The scores of fatigue and fatigue in all dimensions were higher in patients with postoperative chemotherapy for lung cancer.To some extent,8-week mindfulness cancer rehabilitation can effectively relieve the physical fatigue,emotional fatigue,cognitive fatigue and general fatigue of patients undergoing chemotherapy after lung cancer surgery,reduce the level of anxiety and depression,and improve the general self-efficacy and mindfulness of patients,and the effect has a certain degree of sustainability. |
