Abstract/Summary:
Objective:To investigate the effect of mindfulness-based cancer rehabilitation on cancer-related fatigue in patients with bladder cancer undergoing perfusion chemotherapy.Methods:A quasi-experimental study.From March 2022 to December 2022,patients diagnosed with bladder cancer and in need of bladder perfusion in the urology department of a Grade-A hospital in Changchun were selected as research objects through the objective sampling.There were 43 patients in the intervention group and the control group,respectively.The control group received routine nursing,and the intervention group received mindfulness-based cancer rehabilitation intervention on the basis of the control group.Before intervention,general data questionnaire,cancer fatigue scale,hospital anxiety and depression scale and general selfefficacy scale were used to evaluate the effects of the two groups of patients.The effects of mindfulness-based cancer rehabilitation on fatigue,anxiety,depression and general selfefficacy were evaluated immediately after intervention and 3 months after intervention.SPSS24.0 statistical software was used for data analysis.Results:1.General information: The mean age of the patients was 63.84±8.914 years old,most of the patients were male(78.9%),and the tumor stage was mostly T1(60.5%).There was no statistical significance in the general data between the intervention group and the control group(P > 0.05),indicating that the two groups were comparable.2.Analysis results of fatigue: At 3 months ’follow-up after intervention,the scores of total fatigue,physical fatigue and emotional fatigue in the intervention group decreased significantly over time compared with the control group,and the difference between groups was statistically significant(P < 0.001).There was no significant difference in cognitive fatigue between the two groups over time(P=0.072).The time group interaction effect of total fatigue,physical fatigue and emotional fatigue at the three time points was significant(P <0.001),while the time group difference of cognitive fatigue at the three time points was not statistically significant(P > 0.05),and the interaction effect was statistically significant(P <0.001).There were significant differences in the total score of cancer induced fatigue and the scores of each dimension between the two groups at any two time points(P < 0.05).3.Analysis results of anxiety and depression: The results showed that compared with the control group,the HADS scale score of the intervention group decreased significantly over time,and the difference between groups was statistically significant(P < 0.001).There was significant interaction between the two groups of HADS scores at three time points(P < 0.001).The differences in anxiety and depression scores between the two groups were statistically significant at baseline ~ after intervention,after intervention ~ 3 months,and after baseline ~3 months(P < 0.05).4.Analysis results of general self-efficacy: At the 3-month follow-up after intervention,GSES scale scores of the intervention group decreased significantly over time compared with the control group,and the difference between groups was statistically significant(P < 0.001).The intergroup interaction of GSES scores at three time points between the intervention group and the control group was significant(P < 0.001).There were significant differences in GSES scores between the two groups at any two time points(P < 0.001).Conclusion:Mindfulness-based cancer rehabilitation is a safe intervention,which can effectively relieve fatigue symptoms in patients with bladder cancer infused chemotherapy but has no significant effect on cognitive fatigue.It can effectively reduce the anxiety and depression of patients,improve patients’ positive initiative.It is effective and feasible in clinical practice and can be used as an effective measure of psychotherapy for bladder cancer during chemotherapy.
