Effect Of Dexmedetomidine On Remifentanil Induced Hyperalgesia After Operation: Meta-analysis

Objective:To evaluate the effect of dexmedetomidine on remifentanil-induced hyperalgesia after surgery Methods: We searched the English database: EMBASE,The Cochrane Library;the Chinese database: Chinese biomedical database,Wanfang database,Weipu database,China HowNet database,and manually searched the references of related research.To find out a randomized controlled trial of intraoperative dexmedetomidine with placebo or blank control group.Two researchers independently screened literature based on inclusion and exclusion criteria,assessed the quality of research and bias risk,and used RevMan 5.3.5 software for meta-analysis.Results: A total of 19 randomized controlled trials including 1261 cases were included,including 652 cases in dexmedetomidine group and 609 cases in control group.Patients receiving dexmedetomidine reported lower VAS scores at 30 min,1 h,2 h,4 h,8 h and 24 h after extubation than those in the control group(P < 0.05).At 30 min,1 h,2 h,4 h,8 h and 24 h after operation,the VAS scores(0-10)of 95% CI and P were [-1.99(-2.59,-1.4)],[-1.86(-2.4,-1.32)],[-1.68(-2.55,-0.8)],[-1.27(-2.17,-0.36),P<0.05],[-1.87(-3.62,-0.13)],[-1.26(-1.97,-0.54)],respectively.Postoperative complications such as nausea,vomiting,restlessness and shivering were reduced(P < 0.05).There was no significant difference in extubation time between the two groups(P > 0.05).Conclusions: Dexmedetomidine can reduce the incidence of remifentanil-induced hyperalgesia and postoperative complications.
