| ObjectivesStudies showed that CHADS2 score which was used to assess stroke risk in patients with nonvalvular atrial fibrillation was correlated with CAD.This study used GENSINI score to quantitatively evaluate CAD,aiming to explore the correlation between GENSINI score and CHADS2 score in patients with CAD.MethodsThis retrospective study included 606 patients which were suspected with CAD and underwent selective coronary angiography(CAG)during the hospitalization in the department of cardiovascular medicine in our hospital from June 2018 to November 2018.Basic information,past medical history and examination results of the patients were collected.The GENSINI score was calculated based on the coronary angiography results.GENSINI score and the number of diseased coronary arteries were used to quantitatively assess the severity of CAD and correlated with CHADS2.ResultsThe patients included in this study were mainly those over 50 years old(88.8%),and the CHADS2 score was mainly distributed in 0~2 points(91.6%).The CHADS2 score was positively correlated with GENSINI score significantly.The GENSINI score of patients with high CHADS2 score(≥2 points)was significantly higher than that of patients with low CHADS2 score(0~1 points).With the increase of CHADS2 score,the proportion of patients with 3 diseased coronary arteries gradually increased.When the CHADS2 score was over 3 points,all patients had at least 1 diseased coronary artery,and patients with 5 points all had 3 diseased coronary arterries.Age,gender,smoking,white blood cell count,alanine aminotransferase,glutamate aminotransferase,creatinine,uric acid,glycated hemoglobin,high-density lipoprotein cholesterol,triglyceride,heart failure,hypertension,diabetes,CHADS2 score and GENSINI score were significantly different between patients with CAD and without CAD(P<0.05).CHADS2 score(OR=1.433,95%CI 1.094~1.877),age(OR=1.034,95%CI 1.013~1.057),smoking(OR=1.989,95%CI 1.322~2.991),uric acid(OR=1.003,95%CI 1.000~1.005),high-density lipoprotein cholesterol(OR=0.392,95%CI 0.199~0.774),glycated hemoglobin(OR=1.226,95%CI 1.002~1.502)were independent predictors of CAD.CHADS2(OR=1.44,95%CI 0.170~0.561),age(OR=1.03,95%CI 0.010~0.043),smoking(OR=1.67,95%CI 0.192~0.836),creatinine(OR=1.02,95%CI 0.015~0.034),glycated hemoglobin(OR=1.17,95%CI 0.020~0.299),high-density lipoprotein cholesterol(OR=0.33,95%CI-1.662~-0.536)and low-density lipoprotein cholesterol(OR=1.42,95%CI 0.188~0.515)were independent predictors of the number of diseased coronary arteries,and among them high-density lipoprotein cholesterol was the protective factor.ConclusionsCHADS2 score was positively correlated with GENSINI score.It was convenient to evaluate the severity of coronary atherosclerotic heart disease using CHADS2 score with certain clinical significance. |
