Objective: To investigate the relationship between early target organ damage and clinical blood pressure,24-hour ambulatory blood pressure,central aortic pressure in essential patients with mild to moderate hypertension.Methods: 87 patients with mild to moderate essential hypertension in the First Affiliated Hospital of Fujian Medical University were enrolled from March 2017 to January 2019 as hypertension group,and 40 health subjects with normal blood pressure were included as controls.General data of patients were collected,and clinical blood pressure,24-hour ambulatory blood pressure measurements,central aortic pressure(CAP),brachial-ankle pulse wave velocity(baPWV),carotid artery intima-media thickness(IMT),ankle-brachial index(ABI),urinary albumin to creatinine ratio(ACR)and left ventricular mass index(LVMI).Pearson correlation was used to analyze the relationship between early target organ damage and clinical blood pressure,24-hour ambulatory blood pressure measurements and central aortic pressure.The baPWV,IMT,ABI,ACR and LVMI were used as dependent variables,and the other correlative factors were used as independent variables for multiple stepwise linear regression analysis.Results:1.Comparison of clinical blood pressure,the average values of clinical SBP,clinical DBP,24 hSBP,24hDBP,dSBP,dDBP,nSBP,nDBP,24 hDSD,dSSD,nDSD,24 hSCV,24hDCV,CSBP,baPWV,ACR and LVMI in the hypertension group were higher than those in the control group,and the difference was statistically significant(P < 0.05).2.In the hypertension group,(1)Pearson correlation analysis showed that baPWV was correlated with clinical SBP,24 hSBP,dSBP,nSBP,24 hSSD,24hDSD,dSSD,dDSD,nSSD,24 hSCV,24hDCV,dSCV,CSBP(p < 0.05);ABI was not significantly correlated with clinical blood pressure,24 h ambulatory blood pressure and CSBP(P > 0.05).IMT was positively correlated with 24 hSSD,dSSD,nSSD and 24hSCV(P < 0.05).IMT was negatively correlated with 24 hDBP and dDBP(P < 0.05).ACR was correlated with 24 hSSD,24hDSD,dSSD,24 hSCV,24hDCV and dSCV(P < 0.05).LVMI was correlated with 24hDSD(P < 0.05).(2)Multiple linear regression analysis showed that age,clinical SBP and CSBP were the influencing factors of baPWV(p < 0.05).Age and fasting blood glucose are the influencing factors of IMT(P < 0.05).BMI is the influencing factor of ACR(P < 0.05).BMI is the influencing factor of LVMI(P < 0.05).Conclusion:1.Elevated baPWV may be an early target organ damage change in hypertension.;2.Age,clinical systolic blood pressure and central aortic pressure are important risk factors for early atherosclerosis in hypertensive patients;3.In untreated patients with mild to moderate essential hypertension,short-term blood pressure variability has no significant correlation with early target organ damage;4.BMI is an important risk factor for early renal dysfunction and left ventricular hypertrophy in patients with hypertension.
