Research On The Choice Of Policy Tools For The Construction Of Medical Association In Sichuan

With the continuous deepening of medical reform,in order to promote the sinking of high-quality medical resources and the formation of a hierarchical pattern of diagnosis and treatment,all the major cities take the medical association as an entry point to construct the division of labor and cooperation mechanism of different levels and categories of medical institutions.China attaches great importance to the construction of medical association.The government has vigorously promulgated policies on medical association construction to encourage hospitals and community clinics to participate in the construction of medical association.Gradually,medical association has become a research hot spot.Since 2013,medical association has also received more attention from scholars.This topic fuses theory and reality foundation.To give priority to quantitative,qualitative as the auxiliary train of thought.Based on the multiple stream approach,policy tool selection theory,social network analysis method,qualitative data analysis and other research methods,this paper puts forward some suggestions to optimize the policy tool for the development of medical association in Sichuan province from the perspective of policy tool.The research content is organized according to the following logic.Following the research status--the causes of policy formation--digging the existing policy tools--the current situation of medical association construction under the use of existing policy tools--the use of policy tools in the implementation of medical association--suggestions on the use of policy tools.Firstly,this paper adopts the method of literature measurement to clarify the research status of medical association at home and abroad.Then,the formation of medical association policy is analyzed from the perspective of three sources with the multiple stream approach.Then,based on the policy choice theory.Nvivo qualitative analysis software was used to conduct text analysis of 292 collected policies related to medical association from 2006 to 2019,using x-y two-dimensional analysis framework.To discuss the types of policy tools used in the construction of medical association in Chin.And to discuss the emphases and shortcomings of the current policy tools.Furthermore,explore the current situation of medical alliance after the use of current policy tools.Using the social network analysis method and the Ucinet software,the current situation of the cooperation in the medical association database of Sichuan province was analyzed in detail from the macro and micro levels.The caseanalysis method is used to analyze the characteristics of the cooperation of Sichuan provincial people's hospitals and conclude the typical characteristics of the construction of medical association in Sichuan province.Finally,this paper puts forward some constructive suggestions on optimizing the supply policy tools,environmental policy tools and demand policy tools for the cities with different centers in the cooperation network of Sichuan medical association.This topic suggests that in the supply policy tool,strengthen the information construction for the core city and the central city.It is also necessary to strengthen the supporting medical infrastructure construction and attach importance to the service function positioning and financial investment of medical institutions in peripheral cities and peripheral cities.The four kinds of cities should continue to strengthen the use of talent team building tools.Among the demand-oriented policy tools,the government should attach importance to service outsourcing in core and central cities,and strengthen pilot programs.The government should make more use of the fee subsidy tool and enhance the government purchase for peripheral cities and marginal cities.Among the environmental policy tools,it is suggested that the government should strengthen the supervision of the construction of medical association and strengthen the use of propaganda and guidance tools in peripheral cities and marginal cities.To reduce the use of unnecessary publicity and guidance for core cities and central cities,the main focus is to encourage the improvement of performance incentive mechanism.The government should strengthen and improve the policy and system management for the four kinds of cities.
