Objective1.Compare the intestinal flora of patients with peritoneal dialysis-associated peritonitis(PDAP)and non-peritonitis.2.Analysis of peritoneal fluid flora in patients with PDAP.3.Initially explore the correlation between pathogenic bacteria and intestinal and peritoneal fluid in patients with PDAP.It lays a foundation for further studying the relationship between intestinal flora abnormalities and peritonitis in patients with peritoneal dialysis(PD).MethodsThis study is a cross-sectional observational study.From May 2017 to December 2019,non peritonitis patients with peritoneal dialysis(PD group)and patients with peritoneal dialysis related peritonitis(PDAP group)who had stable dialysis in peritoneal dialysis center of Ningxia medical university general hospital for more than three months were included.(1)The demographic data and clinical indicators of all patients were collected,and fecal samples and peritoneal fluid samples of peritonitis patients were collected at the same time.The composition and diversity of intestinal flora of PDAP were determined and analyzed by 16 S RNA amplification sequencing method,and the species difference between groups was analyzed by multivariate statistical method.(2)The data of bacterial culture in peritoneal dialysis fluid of PDAP patients were collected and analyzed with intestinal flora and peritoneal dialysis fluid flora.Results1.30 eligible PD patients and 19 PDAP patients were included.The average age of PD group was 46.33 ± 13.6 years,and the median dialysis age was 21(4,76)months,including 12(40%)males;the average age of PDAP group was 47.68 ± 14.55 years,the median dialysis age was 30(3,92)months,and 12(63.16%)males.In PD group,16(53.3%)had primary glomerular disease,6(20%)diabetic nephropathy,8(26.7%)hypertensive renal damage,9(47.4%)had primary glomerular disease,3(15.8%)diabetic nephropathy,6(31.5%)hypertensive renal damage,and 1(5.3%)polycystic kidney disease.2.Comparison of the diversity and composition of intestinal flora between the PDAP group and the PD group: the t test results showed that ACE,Chao,Shannon and other indicators were not statistically significant(p> 0.05),that is,the diversity of flora ? had no significant difference between the two groups,suggesting No obvious difference between the two groups of species;there was no statistically significant difference in the ? diversity of the two groups of bacteria(p> 0.05),suggesting that there are more common bacteria between the two groups,including Bacteroides,Escherichia coli,F.coli,Clostridium,Prevo Bacteria,Bacteroides and other bacteria,after FDR correction,wilcox test showed no difference.3.Comparison and analysis of fecal and peritoneal dialysis flora in patients with PDAP: t test results showed significant differences in ACE,Chao,Shannon and other indicators(p <0.05),and the diversity of bacterial composition in peritoneal dialysis fluid decreased significantly,suggesting peritoneal dialysis fluid The number of bacteria in a single sample was significantly less than that in fecal flora;there was no statistically significant difference in the ? diversity of the two specimens(p> 0.05).After FDR correction,wilcox test showed that paracoccus,rumenococcus,actinomycetes,haemophilus,etc.were the dominant bacteria in the fecal flora.Lactococcus,acinetobacter,and acetobacter were among the peritoneal fluid flora,Citrobacter,etc.are the dominant bacteria.Compared with the two groups,there was no obvious common dominant flora.4.Correlation analysis of peritonitis pathogens in PDAP patients with intestinal and peritoneal fluid flora: the two most common pathogenic bacteria in PDAP patients: Staphylococcus epidermidis and Escherichia coli and intestinal and peritoneal fluid flora Correlation.(1)The pathogenic bacteria are patients with Staphylococcus epidermidis.The pathogenic bacteria are positively correlated with adjacent Streptococcus granulosus,lactic acid bacteria,and deficient dystrophic bacteria in the intestinal tract and peritoneal fluid.There is a negative correlation between the genus,Clostridium and Clostridium(all p values <0.05);(2)The pathogenic bacteria are patients with Escherichia coli,and the pathogenic bacteria are positively correlated with the ruminococcaceae,Defluviitaleaceae,and aristipse in the intestinal and peritoneal fluid flora.Conclusion1.Compared with non-peritonitis patients,patients with concurrent peritonitis have no significant differences in bacterial group structure and diversity(bacterial group structure,dominant bacterial group).2.The diversity of peritoneal dialysis fluid in PDAP patients was significantly less than that in feces,but no common dominant flora was found between the two.3.The most common pathogenic bacteria Staphylococcus epidermidis and Escherichia coli in PDAP patients are all related to some intestinal and peritoneal fluid flora.
