| ObjectiveUnder the current background of deepening the reform of the medical and health personnel evaluation mechanism,this study analyzes the current situation of the overall quality needs of medical and health personnel,especially surgeons,to understand the comprehensive quality that surgeons need to have.At the same time,it summarizes the current research status and existing problems of the domestic surgeon evaluation system.Based on this,a comprehensive evaluation index system for surgeons in Chongqing is constructed to provide information basis and reference data for future doctor evaluation index system research and hospital personnel management.Methods1.Document Analysis: By referring to Chinese periodical database and English literature database and related websites,to collect relevant literature and data on doctor evaluation in China and abroad.On the basis of extensive reading,it analyzes the emphasis and development direction of domestic and foreign evaluation system for doctors,summarizes the current status and existing problems related to the evaluation system for doctors,and forms alternative index system.2.Expert Interview: Selecting experts from top three hospitals in Chongqing,which are hospital administrators or surgeons,conducting face-to-face interviews with them,and ask them to provide their own insights and suggestions on the content andalternative index of this study,forming a preliminary evaluation index system and an expert consultation form.3.Delphi Method: By issuing expert consultation forms to 36 experts in related fields,two rounds of expert consultations are conducted,and the indexes were selected according to the experts' scoring and suggestions.4.AHP: AHP is used to determine the index weights and improve the evaluation index system.5.Statistical Method: Using Excel2016 to input and analyze statistical data,and using SPSS statistical software to analyze the data.ResultsExpert situation: A total of 36 experts participate in the two rounds of consultation,and the average working life of the experts is(18.0 ± 8.8)years.There are 22 experts(61.1%)with senior titles,10 experts(27.8%)with intermediate titles,and 4 experts(11.1%)with junior titles.Among the experts,there are 18 doctors(50%),11 masters(30.6%)and 7 undergraduates(19.4%)?There are 20 experts(55.6%)engaged in clinical medicine,14 persons(38.9%)in hospital administration,and 2 persons(5.6%)in health management research.The positive coefficients of the experts are 90% and100% respectively,the authoritative coefficient is 0.81,the Kendall coordination coefficients of the two rounds of importance are 0.173 and 0.292,the Kendall coordination coefficients of operability are 0.191 and 0.187,and the significance test is P <0.05.Index results: The index system includes 3 first-level indexes,9 second-level indexes,and 49 third-level indexes.The first-level indexes and their weights are:comprehensive quality 0.283,operational capacity0.643,development potential0.074.The second-level indexes and their weights are: basic quality 0.261,personality ability 0.106,medical ethics 0.633,workload 0.088,medical safety 0.479,work efficiency 0.136,medical quality 0.264,outstanding ability 0.033,academic development 1.000.ConclusionBased on the research purpose,this study uses the Delphi method to select the indexes.Before conducting Delphi consultation,expert interviews are used to ensure the scientific of the consultation form and improve the efficiency of expert consultation.At the same time,Delphi method combined with AHP is used to determine the weight of each index,and a comprehensive evaluation index system for surgeons is constructed.Because the experts have a high degree of professionalism and the research methods used are relatively mature,the final results obtained are reliable,which can provide a thinking direction for the research on the reform of the evaluation mechanism of medical and health personnel. |
