| ObjectiveTo explore the relationship between the severity of coronary artery disease and the risk factors of coronary heart disease(CHD),so as to provide reference for improving the prevention and treatment of the disease.MethodsApply retrospective investigation method to analyze 452 patients undergoing coronary angiography and clinical data of the cases,during January 2012 to January 2013 treated in Department of Cardiology of Shanghai No.9 People’s Hospital.According to the presence or absence of coronary heart disease,the correlation between the number of coronary artery lesions,the size of lesions,the degree of stenosis and the risk factors of coronary heart disease were analyzed.All data were statistically processed using SPSS17.0 software.The measurement data were expressed as mean±standard deviation.The count data were expressed by the composition ratio.Binary Logistic regression and Spearman linear correlation were used to analyze the relationship between different types of coronary artery lesions and various risk factors.Results1.The coronary heart disease group had a younger age,more men,higher education level,and a higher proportion of hypertension and/or diabetes;gender(OR 5.369,95% CI 2.326-12.395,P=0.000),hypertension(OR 0.364,95% CI 0.200-0.661,P=0.001),diabetes(OR 0.446,95% CI 0.212-0.938,P=0.033)were independent risk factors for coronary heart disease.2.In the CHD group,the patients with coronary multi-vessel disease were more older,with lower HDL-C levels,and were associated with hypertension and diabetes;Age(OR 1.045,95% CI 1.013-1.077,P=0.005),diabetes(OR 0.489,95% CI 0.270-0.886,P=0.018)and HDL-C decreased(OR 0.470,95% CI 0.236-0.934,P=0.031)were independent risk factors for coronary multivessel disease.3.The diameter of coronary artery lesions was negatively correlated with age(R=-0.181,P=0.000).The proportion of coronary small vessel disease complicated with diabetes,hypertension and diabetes was high.Male patients with large vascular disease were more likely to have higher uric acid levels;diabetes(OR 1.897,95% CI 1.192-3.021,P=0.007)was an independent risk factor for coronary small vessel disease.4.In patients with severe coronary stenosis,LDL-C levels were significantly elevated and left ventricular ejection fraction was significantly reduced;LDL-C(OR 1.329,95% CI 1.033-1.708,P=0.027)was an independent risk factor for severe coronary stenosis;The degree of coronary stenosis was positively correlated with LDL-C(R=0.141,P=0.000)and heart rate(R=0.133,P=0.009),and negatively correlated with LVEF(R=-0.213,P=0.000).ConclusionAge,gender,hypertension,diabetes,education level,hypertension and diabetes are correlated with the incidence of coronary heart disease.Aging,diabetes,hypertension and diabetes,lower HDL-C,higher LDL-C,higher uric acid and higher heart rate will aggravate the severity of coronary artery disease.Left ventricular function decreased with the increase of coronary artery stenosis in patients with coronary heart disease. |
