Objective:By retrospectively studying the expression levels of different types of serum tumor markers in interstitial lung disease?ILD?,analysising the correlation between serum tumor markers and ILD types and disease severity,and to explore the clinical evaluation and diagnostic value of different serum tumor markers in ILD.Methods:Retrospective collection of clinical data of 151 patients diagnosed with ILD who attended the First Hospital of Jilin University from January 2016 to December 2018,including gender,age,smoking history,serum C-reactive protein?CRP?,arterial blood gas without oxygen inhalation at admission,pulmonary function and other data,and serum tumor markers detected under fasting state after admission were recorded,include:alpha-fetoprotein?AFP?,carcinoembryonic antigen?CEA?,neuron-specific enolase?NSE?,cytokeratin 19 fragment?Cyfra21-1?,squamous cell carcinoma antigen?SCCA?,carbohydrate antigen 125?CA125?,carbohydrate antigen 199?CA199?,carbohydrate antigen 724?CA724?,carbohydrate antigen 242?CA242?and increased recording of carbohydrate antigen 153?CA153?in female.We divide ILD patients into two subgroups according to clinical diagnosis:idiopathic interstitial pneumonia?IIP?group,connective tissue disease-interstitial lung disease?CTD-ILD?group,at the same time,45 patients who were simply diagnosed as community-acquired pneumonia were randomly selected as clinical control group.The pO2 index of arterial blood gas analysis without oxygen inhalation at admission was used as the standard for judging the severity of disease in patients with ILD.According to the expression levels of different serum tumor markers in the ILD group and its subgroups,determine whether there is a difference between the expression of tumor markers and the type of ILD,and analyze the correlation between the serum tumor markers and the severity of ILD.In the ILD group and the control group,the receiver operating characteristic curve was used to analyze the critical value of different serum tumor markers in the diagnosis of ILD,and to explore the sensitivity and specificity of different serum tumor markers in the diagnosis of ILD.Results:?1?In the analysis of serum tumor markers in the ILD subgroup,the expression levels of CYFRA21-1 and CEA in the IIP group are higher than those in the CTD-ILD group,and other serum tumor markers are not significantly different in the two subgroups;?2?Through ROC curves analysis of different serum tumor markers in the diagnosis of ILD,Cyfra21-1,CA125,CA199,CA242,and CA153 have AUC values greater than 0.7.Among them,CA125 has the highest value in the diagnosis of ILD,and the AUC value is0.811.?95%confidence interval:0.737-0.884?,when the critical value is8.72U/ml,the sensitivity is 88.8%and the specificity is 62.2%;?3?The pO2value in the blood gas analysis measured without oxygen inhalation is used as an index to judge the severity of lung damage,correlation analysis of the pO2value and serum tumor markers CA125,Cyfra21-1,CA199,CA242 and CA153 with high sensitivity and specificity in the diagnostic value of ILD.CA125,Cyfra21-1 and CA199 are negatively correlated with pO2.Among them,CA125 and CA199 have a strong correlation?correlation coefficients are:r=-0.449,r=-0.343?.Conclusions:?1?The serum tumor markers,such as Cyfra21-1,SCCA,AFP,NSE,CA199,CEA,CA242,CA125 and CA153,having increased expression levels of patients with ILD,and the expression levels of Cyfra21-1and CEA have certain value in the differential diagnosis of IIP and CTD-ILD.?2?In the diagnosis of ILD,the serum tumor markers with high sensitivity and specificity are Cyfra21-1,CA125,CA199,CA242 and CA153,and the best diagnostic value is CA125.?3?Cyfra21-1,CA125 and CA199 are negatively correlated with the level of pO2 in the blood gas analysis of ILD,and it is of certain clinical value to evaluate the disease severity of ILD patients by its expression level.
