.94 Cases Of Interstitial Lung Disease In Patients With Tcm Syndrome Analysis And Use Of The Experimental Study Of The Tongbi Synthetic Treatment Of Interstitial Lung Disease

Interstitial lung disease(ILD) are a group of major interstitial lung involvement, alveolar and(or) diffuse bronchioloalveolar lung disease.Interstitial lung disease pulmonary fibrosis is the final pathology.Its etiology and pathogenesis are still not fully clear,progressive disease was increase in general,the incidence rate showed an upward trend in recent years,serious harm,there is currently no effective cure,therefore the research on the ILD has become a global hot spot of common concern.This paper reviews the literature in recent years,the theoretical basis for ILD research, traditional Chinese and western medicine syndrome differentiation and treatment of the study.That the lack of clinical syndromes on the ILD system of law,animal experiments to explore traditional Chinese medicine on coagulation and fibrinolysis imbalance in ILD and produce collagen fibers and promote the adjustment factor to improve lung function,is the treatment of ILD in a new direction of development.Part One:94 cases of interstitial lung disease in patients with TCM Syndrome AnalysisObjective:To investigate the etiology and pathogenesis ILD,interstitial lung disease research of TCM Syndrome distribution,try to find some syndromes classification rules to provide a treatment based on the differentiation.Methods:A cross-sectional survey approach,the development of the clinical questionnaire,detailed history of data collection;Analysis of disease due to sexually transmitted disease-bit machine;refer to Diagnosis and Treatment of instructor experience, the evidence-based national standards and "Chinese medicine differential diagnosis Syndrome" Symptoms Differentiation measurement points form a comprehensive analysis of the corresponding card type;the establishment of databases,the use of SPSS13.0 analysis software,statistical distribution of ILD major syndromes,analysis of evidence-based blood stasis and pulmonary function and the relationship between test results.Results:94 cases of ILD in patients with major elements syndrome:disease in the lungs,spleen,kidney,heart,Qi-deficiency,yin deficiency,yang deficiency,blood stasis, phlegm,moisture,heat,wind devil.Evidence-based distribution:the evidence:both Qi and Yin Deficiency＞Lung Qi Deficiency＞Yin Deficiency＞Yang Deficiency;and folder card: Blood Stasis＞heat obstruct lung＞wind devil invade lung＞dampness pulmonary resistance. In all card-based combination of methods,both Qi and Yin deficiency and blood stasis syndrome in patients is the most.Deficiency accounts for 43.62%of patients,inclusion of patients with actual situation accounted for 56.38%.Card folder and appear in the frequency of blood stasis and increase the degree of deficiency was a significant positive correlation.Lung function in this case the evidence showed:Compared with Yin Deficiency patients,both Qi-Yin Deficiency and Yang Deficiency Syndromes patients showed a significant decreased tendency towards of FVC content;Yang Deficiency patients showed a significant decreased tendency towards of TLC content compared with Yin Deficiency patients and Qi Deficiency patients;Compared with Yin Deficiency,patients of Qi-Yin Deficiency and patients of Yang Deficiency showed a significant decreased tendency towards of TLC content.The blood-gas analyses showed:Compared with Yin Deficiency patients,Yang Deficiency patients showed a significant increased tendency towards of TLC P (A-a) O₂.Conclusions:ILD major card type as follows:the evidence,including Qi-Yin Deficiency,Qi Deficiency,Yin Deficiency,Yang deficiency;and folder card,including: blood stasis,heat obstruct lung,wind devil invade lung,phlegm lung resistance. Multi-folder-based and marked deficiency in evidence,Qi-Yin deficiency with blood stasis pattern is the most;in the start,it invades the lung,then invades the spleen and kidney,in the end,it involves the heart,both yin and yah loss of organs;The deficiency is the main aspect,which including Yin deficiency,Qi-Yin Deficiency,Yang deficiency.TCM syndrome distribution and lung function and blood-gas analysis indicators that certain relevance may exist between them,this study may provide some help for the objective of the TCM syndrome, the specific relationship between them need to be further studied. Part Two:Experimental Study on the effect of Tongbihuaxianfang for interstitial lung diseaseObjective:To investigate the mechanism of Tongbihuaxianfang in treating ILD and its effect in pulmonary function.Methods:70 male SD rats,10 rats were randomly selected intratracheal injection of normal saline as normal control group,the remaining 60 intratracheal bleomycin injection resulted in pulmonary fibrosis model of rat.in the first 28 days after be modeled,the survival model rats were randomly divided into model group,estrogen group,the Chinese Medicines group,a total of 3 groups.Experimental animals in each group were administered 28 days after delivery,28 days for delivery.After the first 56 days,and pulmonary function in rats forced(FVC) test to measure FVC,FEV0.3/FVC%,PEF,MVV,Cdyn and so on.After pulmonary function obtained in bronchoalveolar lavage fluid,ELISA detection of uPA,PAI-1 content.The main abdominal vein blood,serum enzyme uPA,PAI-1 levels,RIA of serum TNF-αcontent.Arterial blood rapidly after the removal of the lungs,the naked eye to observe the appearance of lung tissue patterns;select the left lower lobe of rats the same site for biopsy,for HE staining to observe morphological changes.Results:(1) Pulmonary fibrosis model was successfully established.(2) TNF-αlevels:Compared with the normal group,the levels of TNF-αin model group was significantly higher,and it is significantly higher than the Chinese Medicines Board and hormone group.(3) The content of uPA,PAI-1 from Serum and BALF:Compared with the normal group,the model group,the Chinese Medicines Board and hormone serum levels of uPA were significantly reduced.(4) Pulmonary function indicators:Model group rats were have a significantly lower FVC;rats FEV0.3,FEF75 and MVV significantly lower in model group and normal group;Cdyn in model group has a morn significantiy decreased tendency than normal group;For both RL and RE level,model group has a significantiy increased tendency,compared with normal group;The RL levels of Chinese Medicines Board is significantly lower compared with the model group.Conclusion:Tongbihuaxianfang may delay the process of fibrosis mechanism by lowering serum levels of TNF-α.In this study,If the mechanisms of imbalance between coagulation and fibrinolysis is existent,as well as Tongbihuaxianfang improve ILD by regulating the imbalance between coagulation and fibrinolysis,which require further study. ILD model of rat lung function showed a restrictive ventilatory dysfunction and Cdyn reduced, RL increased.Tongbihuaxianfang can improve Cydn and RL.
