Abstract/Summary:
Background:The purpose of this article is to investigate the effect of neostigmine on postoperative pulmonary complications after laparoscopic surgery in elderly patients.Method:120 patients undergoing elective general anesthesia laparoscopic surgery, aged 65-75 years, grade II or III of the American Association of Anaesthetists. All patients were randomly divided into 2 groups of 60 people each: the neostigmine group(group N)and the control group(group C).When the TOF count reached 2,patients in group N were given neostigmine 0.04 mg / kg and atropine, and the amount of atropine was 1/2 of the dosage of neostigmine.Patients in group C were given saline. After the patient’s consciousness, cough reflex after swallowing, and muscle strength recovered, the tracheal tube was removed.Observation indicators: the general situation of the two groups of patients and the basic situation during the perioperative period.Postoperative adverse reactions included sweating, arrhythmia, respiratory depression, itching of the skin, and nausea and vomiting,Steward’s wake score, and Ramsay’s sedation score.Postoperative visits at 3 days included fever, hypoxemia, abnormal auscultation of the lungs, and satisfaction with anesthesia.Patients’ TOF ratio(a train of four ration,TOFr)distribution in the awake room(PACU),the proportion of residual neuromuscular block, the incidence of postoperative pulmonary complications 7 days after the operation, and the hospitalization within 30 days And death.Test results:There were no statistically significant differences in the general information before surgery and the basic conditions during the perioperative period. However, the hospitalization time of patients in group C was significantly longer than that in group N,and the difference was statistically significant(P <0.05).There were no significant differences inpostoperative adverse reactions. There were no significant differences in fever, hypoxemia, abnormal auscultation in the lungs, and satisfaction with anesthesia at the 3rd postoperative visit. When the patients were in the recovery room, the proportion of residual neuromuscular block after operation in group N was significantly lower than that in group C(P <0.05).Postoperative pulmonary complications occurred in 18 of the 120 patients, including 6 cases of pneumonia,5 cases of hyoxemia,4 cases of atelectasis, and 3 cases of bronchospasm. No other postoperative pulmonary complications were found. The incidence of postoperative pulmonary complications was lower in group N,and the difference was statistically significant(P <0.05)Conclusion:In this study, for elderly general anesthesia patients, neostigmine can effectively reduce the incidence of postoperative pulmonary complications after laparoscopic surgery。...
