Objective:To explore the safety and application value of early enteral nutrition management in patients with rectal cancer after radical operation.Method:From August 2018 to October 2019,83 patients undergoing radical resection of rectal cancer in our hospital were randomly divided into experimental group and control group.In the experimental group(n = 44),the early enteral nutrition(EEN)+ parenteral nutrition(PN)management mode was implemented.In the control group(n = 39),the total parenteral nutrition(TPN)management mode was implemented.All of the patients were confirmed as rectal cancer by endoscopic biopsy and pathology before operation.The general condition,nutritional index(TP,ALB,PAB),immune index(C3,C4,IgA,IgG,LYM),inflammatory index(CRP,WBC),postoperative rehabilitation index(time of first postoperative exhaust,time of first postoperative defecation,days of hospitalization,total cost of hospitalization),adverse reactions or complications(nausea and vomiting,abdominal distention,diarrhea,lung infection,incision infection,anastomotic leakage)were recorded and compared between the two groups.Result:1.Comparison of general conditions: there was no significant difference between the two groups in gender,age,preoperative NRS 2002 score,preoperative nutritional risk,combined basic diseases(hypertension and diabetes),operation mode,tumor TNM stage and pathological tissue type(P > 0.05).2.Comparison of nutritional indicators: there was no significant difference in TP,ALB and PAB levels between the two groups on the first day before operation(P > 0.05).After operation,the levels of TP,ALB and PAB in the experimental group were higher than those in the control group on the 1st,3rd and 7th day after operation,the difference was statistically significant(P < 0.05).3.Comparison of immune indexes: before operation,there was no significant difference in C3,C4,IgA,IgG and LYM levels between the two groups on the first day before operation,all P > 0.05.After operation,the levels of C3,C4,IgA,IgG and LYM in the experimental group were higher than those in the control group on the first and third days after operation,the difference was statistically significant(P < 0.05).4.Comparison of inflammatory indexes: before operation,there was no significant difference in CRP and WBC level between the two groups(P > 0.05).After operation,the levels of CRP and WBC in the experimental group were lower than those in the control group on the 1st,3rd and 7th day after operation,the difference was statistically significant(P < 0.05).5.Comparison of postoperative rehabilitation indicators: after the operation,the experimental group was significantly less than the control group in the time of the first postoperative exhaust,the time of the first postoperative defecation and the time of hospitalization,the difference was statistically significant(P < 0.05).There was no significant difference in the total cost of hospitalization between the two groups(P > 0.05).6.Comparison of postoperative adverse reactions or complications: there was no significant difference in nausea and vomiting,abdominal distention,diarrhea,postoperative intestinal obstruction,anastomotic leakage,lung infection and incision infection between the two groups(P > 0.05).Conclusion:1.Early enteral nutrition management can improve the nutritional status and immune status of patients,reduce the intensity of postoperative inflammatory response,and promote postoperative recovery.2.The early enteral nutrition management mode is safe and reliable,and does not increase the risk of adverse reactions or complications.
