Objective:A new type of 14C breath test solid scintillation bottle was developed to detect the efficacy of Helicobacter pylori.Methods:In the study,289 physical examinees were selected in the physical examination center of our hospital from December 2018 to December 2019,and 123newly diagnosed patients with upper abdominal discomfort in the endoscopic center of Affiliated Hospital of Zunyi Medical University,a total of 412 people.All patients were tested for Helicobacter pylori using both the 14C solid scintillation vial and the14C breathing card Urea breath test method.Firstly,the physical examination group was divided into A and B groups.In group A,a solid scintillation bottle method was used first,and then breath card method was used;in group B,it was the opposite.Through comparison,the influence of sampling interval time on the results is analyzed.In the endoscopic group,123 patients underwent biopsies,and 41 of them underwent immunohistochemistry;Because some tissue samples were too small,real-time quantitative PCR was performed on the tissue samples of 82 patients as two comparative standards for detecting Hp infection.The sensitivity,accuracy,and stability of Helicobacter pylori were detected by solid scintillation vial method and compared with the expiratory card method.Results:?1?In the physical examination group,the positive rate of solid scintillation UBT in group A was 13.89%?20/144?,and the positive rate of breath-card method UBT was 15.17%?43/144?;the positive rate of solid scintillation in group B was7.64.%?20/145?,the positive rate of the breath-card method test was 4.14%?22/145?,and there was no statistically significant difference between the positive rates of the two;?2?In the immunohistochemistry group,the positive rate of IHC was 39.02%?16/41?,the positive rate of solid scintillation vial method in this group was 29.27%?12/41?,and the positive rate of the breath-card method was 31.71%?13/41?.There was no statistically significant difference in the positive rates of the two UBTs?P>0.05?.Taking immunohistochemistry as the gold standard,the sensitivity of the solid scintillation bottle is 91.67%,and the specificity is 96.00%;the sensitivity of the breath-card method:92.31%and the specificity:96.00%;?3?In the PCR group,the positive rate of PCR was 35.37%?29,82?,the positive rate of Solid scintillation UBT test was 29.27%?24/82?,and the positive rate of breath-card method UBT was37.80%?31/82?.Taking PCR as a gold standard,the sensitivity of Solid scintillation UBT is 82.76%,and the specificity is 100.00%;the sensitivity of the breath-card method UBT is 86.21%,and the specificity is 88.46%;?4?The accuracy of solid scintillation bottle UBT is 93.90%?77/82?,the false positive rate is 0,and the missed diagnosis rate is 17.24%?5/29?;the accuracy of the breath-card method UBT is81.81%?72/82?,the false-positive rate was 11.32%?6/53?,and the missed diagnosis rate was 13.80%?4/29?.There was no statistically significant difference in the sensitivity and missed diagnosis rate of the two UBTs?P>0.05?.Comparing the specificity,accuracy and false positive rate of the two UBTs,the difference was statistically significant?P<0.05?.?6?The area under the ROC curve for solid scintillation-style diagnosis of Hp infection was 0.966.The area under the ROC curve for the breath-card method assay to diagnose Hp infection was 0.934.The difference in the area under the ROC curve between the two was not statistically significant.Conclusions:?1?Within a certain time range,the positive rates of the two UBT were not affected by the sampling interval;?2?The UBT of solid diagnosis meets the requirements of clinical detection of HP infection and can be used in clinical practice;?3?The sensitivity of UBT based on solid scintillation is similar to that of breath card method UBT.However,in terms of specificity,accuracy and misdiagnosis rate,solid diagnosis UBT is superior to breath card method UBT;?4?The results show that the stability of the solid diagnosis UBT is the same as that of the breath card method UBT;?5?The infection rate of UBT was faster than that of breath card method.
