Objective:To observe the clinical effect of acupoint embedding intradermal needle combine with oral mosapride on diabetic gastroparesis(DPG),gastric emptying,blood glucose and gastrointestinal hormones.Further to investigate its mechanism in the treatment of diabetic gastroparesis and evaluate its safety and effectiveness.Methods:A total of 90 patients with DPG were randomly divided into three groups A,B and C(30 patients in each group).Before enrollment,all patients stopped hypoglycemic drugs(such as metformin and acarbose)which may cause side effects on gastrointestinal tract for 1 week and replaced with other oral hypoglycemic drugs or insulin for blood glucose control.Group A took 5mg(1 tablet)mosapride orally for3 times/day,30 minutes before meals,for 4 weeks.Group B combined with traditional acupuncture acupoint on the basis of group A once a day.Group C combined with acupoint embedding intradermal needle on the basis of group A once a day.Group B and group C were all under treatment with acupoint of Zhongwan(RN12),shangwan(RN13),zusanli(ST36),sanyinjiao(SP6),tianshu(ST25)and neiguan(PC6)for 4 weeks.The clinical efficacy and overall symptom scores of the three groups were observed before and after treatment.Gastric emptying rate(GER)and gastric emptying time(GET)were measured by gastric ultrasound.Levels of fasting blood glucose(FBG),2hpostprandial blood glucose(2hPG),glycosylated hemoglobin(HbA1c)and motilin(MTL),gastrin(GAS)and somatostatin(SS)were tested before and after therapies.The side effects of treatments were recorded during the whole study.Results:(1)Symptom scores decreased significantly in all groups after treatment than before with statistically significant difference(P<0.05).After treatment,compared with group A,symptom score in group B and group C decreased and effective rate oftreatment increased obviously(P< 0.05).But there was no significantly difference in symptom score and effective rate between group B and group C(P>0.05).(2)The GER at 30 min and 60 min were increased significantly after treatment in all three groups than before(P<0.05),the GET decreased significantly(P<0.05).After treatment,compared with group A,the GER of 30 min and 60 min increased,the GET decreased significantly in group B and group C(P<0.05).After treatment,there was no significantly difference between group B and group C in 30 min GER,60 min GER and GET(P>0.05).(3)FBG,2hPG and HbA1 c were decreased significantly in all three groups after treatment than before,with statistically significant differences(P<0.05).After treatment,compared with group A,FBG,2hPG and HbA1 c decreased significantly in group B and C(P<0.05).After treatment,there was no significantly difference in FBG,2hPG and HbA1 c between group B and group C(P >0.05).(4)Compared with before treatment,MTL and GAS decreased,while SS increased significantly after treatment in the three groups(P<0.05).After treatment,compared with group A,MTL and GAS decreased while SS increased significantly in group B and group C(P<0.05).But with no significantly difference between group B and group C(P>0.05).(5)During the treatment periods,none of drugs side effects were observed,such as rash,fatigue,palpitations,or broken needle or hematoma.Conclusions:1.Acupoint embedding intradermal needle improved the clinical effect obviously of DGP patients by reduce the overall symptom scores,increase gastric emptying rate and reduce gastric emptying time.Along with better control in gastro-intestinal hormone levels and blood glucose.2.The effects on DGP with acupoint embedding intradermal needle combine with oral mosapride was better than oral mosapride alone.It suggested integrated Chinese and western medicine has unique advantages over western medicine alone.3.Acupoint embedding intradermal needle and traditional acupuncture point was similarly in the treatment of DGP.The underline mechanism may contributed toregulation of gastrointestinal motility,blood glucose and improvement of gastrointestinal hormone levels by continuous stimulation of acupoints.
