Objective:Based on the monitoring data of human brucellosis in Jiangxi Province from 2012 to 2018,to grasp the epidemiological characteristics and dynamic changes of the brucellosis epidemic situation in Jiangxi Province from 2012 to 2018,and to monitor the distribution of cloth acquired during 2017-2019 The analysis of genetic characteristics of Brucella Strains,through genotyping and cluster analysis,grasp the dominant strains and genotypes in our province,and provide a reference for future prevention and control work.Methods:(1)Collect the human Brucella cases(including clinical diagnosis and laboratory diagnosis cases)in Jiangxi Province from January 2012 to December 2018 through the Chinese disease prevention and control information system(statistics by reporting area and date of onset)and by sex Resident information by year,year and age.Descriptive analysis of the epidemic data among patients with brucellosis in Jiangxi Province from 2012 to 2018 was carried out to describe the epidemic situation,regional distribution,population distribution and time distribution of brucellosis in this period.(2)Collect venous blood specimens of suspected cases and surveillance subjects from January 2018 to January 2019 through case monitoring,use of multi-site variable number tandem repeat sequences Analysis(MLVA-16)technical analysis carried out typing and cluster analysis of Brucella isolates,combined with on-site epidemiology,and conducted a province-wide molecular epidemiology traceability survey.Results:(1)A total of 331 cases of brucellosis were reported in Jiangxi Province from 2012 to 2018,with an incidence rate ranging from 0.02 / 100,000 to 0.2 / 100,000,with no deaths.The time of onset was concentrated from May to September.The incidence was the lowest in February throughout the year.In the past 7 years,the cases were distributed in 74 counties in 11 prefecture-level cities in the province,mainly in the central and northern Jiangxi.The top three prefecture-level cities with reported cases are Nanchang,Shangrao,Jiujiang,and Fuzhou.The top three counties(districts)with reported cases are Xinjian County,Yushui District,and Nanchang County.Among the cases,there are 244 males and 87 females,and the average annual incidence of males is higher than that of females;the age is concentrated in the age group of 40-64 years,with a minimum of 1 year old and a maximum of 79 years old;Mainly,the dynamic characteristics of occupational distribution show that the proportion of non-professional population has increased from 30% in 2015 to 63.4% in 2016.(2)In 2012-2018,a total of 11 cases of brucellosis were reported,affecting 3041 people and 22 cases with an attack rate of 0.72%.Among them,there were 5 cases in Nanchang City,1 case in Ganzhou City,Pingxiang City,Shangrao City,Yingtan City,Ji'an City and Jingdezhen City.Covering 11 towns(streets),8 towns and 3 street(district)roads.6 cases of vector animal and plant transmission,3 cases of life contact,1 case for unknown reasons,1 case for other reasons(3)Since the first serological monitoring of brucellosis in key populations was carried out in Xinyu City in 2012,Jiangxi Province has collected 7160 serum samples,95 of which were positive by SAT test,with a positive rate of 1.33%.Among them,the positive rate(4.41%)was the highest in 2014.Since 2013,the etiology monitoring of brucellosis has been carried out.From 2013 to 2017,62 strains of brucellosis were isolated from the blood of 193 cases.58 strains of sheep were identified(including 4 sheep I and 54 sheep III)and pig I 4 strains.(4)Two strains were unable to perform cluster analysis due to incomplete amplification product results.MLVA clustered the remaining 44 strains into sheep type 3.MLVA-8 showed that 44 strains of Brucella contained 4 genotypes,with type 42 as the dominant genotype,and there were differences in Bruce11 and Bruce43 loci.MLVA-11 clustered 44 strains of sheep seed bacteria into 10 genotypes,with(1-5-3-13-2-2-3-2-4-20-7)as the dominant genotype,panel 2A group There are certain differences in the three sites.MLVA-16 showed 32 genotypes,of which 10 genotypes were shared genotypes and 22 genotypes were single isolates.Panel 2B Bruce04,Bruce16 and Bruce30 loci showed a high degree of variability.There are 5 VNTRs in Bruce07,and there is no difference in Bruce09 in all strains.(5)In the 43 cases,there are more men than women,concentrated in the age of 40-59 years old,and the occupational distribution is mainly farmers.There were 41 people in the province and 2 people outside the province.Most patients have a history of animal contact,and the most exposed animals are sheep.The contact history is mainly breeding,slaughtering,and selling animal products.The unknown contact history accounts for 16.3%.There were no epidemiological links between shared genotype cases isolated from the same area.At the same time,strains jx25,jx32 and jx41 were isolated from the same cluster of epidemic cases,but the three did not share the same MLVA-16 type.Conclusions:(1)The cases are sporadic,and they are mainly young and middle-aged male farmers.The form of transmission is diversified,mainly by direct contact,and some food-borne infections or unexplained causes.(2)The dominant bacterial type causing Brucellosis in Jiangxi Province is Brucella mutton type 3.MLVA-8,MLVA-11 and MLVA-16 produced 3,9 and 32 genotypes,respectively.and genotype 42 is the main type in MLVA-8 typing.MLVA-11 typing is(1-5-3-13-2-2-3-2-4-20-7)as the dominant genotype,and MLVA-16 typing shows the height at Bruce04,Bruce16 and Bruce30 Variability.(3)MLVA-16 cluster analysis makes up for the lack of traceability caused by the presence of intermediate hosts in field epidemiological investigations.
