| Objective:To investigate the effect of body mass index(BMI)on luteinizing hormone(LH)peak after gonadotropin-releasing hormone(GnRH)stimulation test in girls with central precocious puberty and the relationship between other indicators of sexual development Methods:From December 2017 to December 2019,110 girls with central precocious puberty were selected from the gonadotropin-releasing hormone stimulation test at the department of pediatrics,The Second Affiliated Hospital of Nanchang University.They were divided into normal weight group(n = 45),super recombination group(n = 35)and obesity group(n = 30)according to body mass index.Physical examination of the girl: height,weight,breast and pubic hair development;Hormone measurements: luteinizing hormone(LH)basal value,follicle-stimulating hormone(FSH)basal value,estradiol(E2),testosterone(T),GnRH stimulation test;Blood lipid test: triglyceride(TG),total cholesterol(TC),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C).Insulin-like growth factor-1(IGF-1);Bone age(BA)was measured,uterine and ovarian ultrasound was performed,and BMI was calculated.SPSS20.0 software was used for data processing and analysis.One-way anova,LSD-L,Kruskal Wallis H test and Spearman correlation analysis were used.Results:General information: there was no statistical difference in the average age of diagnosis between the three groups(P>,0.05),but the difference in BA and ba-ca was statistically significant(P<0.05).The height,weight,BMI,height-sds,weight-sds and bm-sds of girls in the three groups were statistically significant(P<0.05).There were statistically significant differences in the distribution of breast and pubic hair development after Tanner stage in girls with central precocious puberty in the three groups(P<0.001).There were no statistically significant differences in the average size and maximum diameter of bilateral ovaries in the three groups(P>0.05).Serum lipid and IGF-1: TC and HDL-C were not statistically significant in each group(P>0.05),while TG,LDL-C and IGF-1 were statistically significant(P<0.05).Sex hormones: there was no statistically significant difference in the peak values of LH base value,FSH base value,E2,T and FSH between the three groups(P>0.05),and the difference of LH peak value and LH/FSH peak value after GnRH excitation test was statistically significant(P<0.05).Correlation analysis of BMI and other indicators: there was no correlation between BMI and average ovarian size,maximum follicle diameter,LH base value,FSH base value,LH peak value,E2,T,TC and HDL-C of girls.It was positively correlated with bone age,ba-ca,TG,LDL-C and IGF-1.It is negatively correlated with the peak value of LH and the peak value ratio of LH/FSH.Conclusion:Obesity can accelerate bone maturation,and obese girls have dyslipidemia,so their weight should be controlled timely.The effect of BMI on LH peak and should be taken into account when analyzing the results of GnRH stimulation test in girls. |
