Objective:To investigate the correlation between serum bilirubin,serum uric acid and c-reactive protein levels and acute exacerbation of COPD and the comprehensive evaluation of COPD.Methods:Data of 115 patients hospitalized with AECOPD in the first affiliated hospital of shihezi university from October 2017 to June 2019 and their cases entering the stable period after 10-14 days of treatment were collected.The level changes of TBIL,UA and CRP values in AECOPD group and COPD group were described by self-matched sample t test.The correlation between TBIL,UA,CRP and acute exacerbation of COPD was analyzed by binary logistic regression.ROC curve judged whether the value of the combined prediction of AECOPD by TBIL,UA and CRP was better than a single index,and calculated the optimal cut-off points of the three.Pearson correlation was used to analyze the correlation of TBIL,UA,CRP and WBC,NEU%,PaO2 and SaO2 in AECOPD stage.mMRC dyspnea index,CAT COPD assessment test and acute exacerbation history were used to divide the patients into groups A,B,C and D after entering the COPD stabilization period,and the lung function of patients with COPD stabilization period was divided into grade I-IV with GOLD grading.ANOVA test analysis was used to compare the level changes of TBIL,UA and CRP in the ABCD group and grade I-IV group.Pearson correlation was used to analyze the correlation between TBIL,UA and CRP values and mMRC and CAT scores in stable period.Results:Paired t test showed that the values of TBIL,UA and CRP in AECOPD group were higher than those in the stable period group?all P<0.05?.Binary logisitic regression analysis showed that TBIL,UA and CRP all affected the occurrence of AECOPD?all P<0.05?.The area under the ROC curve of AECOPD was greater than a single index,and its joint prediction value was better than a single index.ANOVA test showed that TILB and UA basically gradually decreased and CRP gradually increased in the A-D group during the stable period of COPD.The levels of TILB,UA and CRP in the stable period of COPD did not change significantly in the pulmonary function classification.Pearson correlation analysis showed that TBIL in AECOPD group was negatively correlated with PaO2 and SaO2?r values were-0.26 and-0.35,respectively,P<0.05?.UA was negatively correlated with PaO2 and SaO2?r values were-0.30 and-0.45,P<0.05?.CRP was positively correlated with WBC?r value was 0.31,P<0.05?.TBIL at the stable stage of COPD was negatively correlated with CAT and mMRC?r value was-0.21,-0.24,P<0.05?.UA was negatively correlated with CAT and mMRC?r values were-0.37 and-0.33,respectively,P<0.05?.WBC was positively correlated with CAT and mMRC?r value was 0.69,0.66,P<0.05?.Conclusions:1.Serum bilirubin,serum uric acid and CRP had good consistency in predicting the occurrence of AECOPD,and the combined diagnostic value of the three was greater than a single index.2.In AECOPD,serum bilirubin and serum uric acid can indirectly assess the severity of hypoxia,and CRP and WBC have a certain value in the combined assessment of infection degree.3.Serum bilirubin,serum uric acid,and CRP may be used as the objective basis for the evaluation of COPD stabilization,mMRC questionnaire,and CAT questionnaire.
