Objective:To understand the current situation of imaging equipment and imaging health personnel allocation in township health centers in Ganzhou City,and to comprehensively evaluate the existing imaging equipment and to analyze the related factors affecting the use effect of equipment.The purpose of this study is to provide reference and suggestions for further optimizing image resource allocation and improving medical image service ability in township health centers.Methods:Using random cluster sampling method,56 township health centers(20 central hospitals and 36 general hospitals)were selected from 18 counties and cities in Ganzhou City as research objects.A self-designed questionnaire was used to investigate 56 township health centers on the spot.Stratified random sampling method was used to randomly select 4 and 6 hospitals from 20 central hospitals and 36 general hospitals respectively.Personal interviews were conducted with the presidents,administrative personnel,clinicians and imaging personnel of 10 township health centers.The quantitative data are established by EXCEL2007 and analyzed by SPSS20.0 statistical software.T test was used for comparison the measurement data between two groups,chi-square test was used for comparison of composition ratio or rate,stepwise regression analysis was used for analysis of influencing factors of use effect,and the test level was = 0.05.Results:1.Configuration of imaging equipment: The existing medical imaging equipment in township health centers mainly includes X-ray equipment and ultrasonic diagnostic equipment,including common X-ray equipment(25.34%),CR(10.00%),DR(8.00%),CT(3.33%),black-and-white ultrasound(33.33%)and color Doppler ultrasound(20.00%).56 township health centers have 2.68±0.89 sets of imaging equipment,with domestic equipment accounting for 86.67%.DR(17.18%)and color Doppler ultrasound(29.69%)are the main X-ray equipment in the central hospitals,while ordinary X-ray machines(30.58%)and black-and-white ultrasound(41.18%)are the main ultrasonic diagnostic instruments in the general hospitals.The number of imaging equipment in the central hospitals(3.20±1.69)is higher than that in the general hospitals(2.39 ±1.26)(P<0.05).2.Service life of imaging equipment: the average service life of imaging equipment in township health centers is 6.31±4.19 years,the average service life of X-ray equipment in central hospitals(3.75±4.06 years)is less than that in general hospitals(7.87± 4.09 years)(P value is less than 0.05),and the equipment with high overdue use rate is common X-ray machine(92.59%)and black-and-white ultrasound(54.76%)3.Use efficiency of imaging equipment: The proportion of imaging equipment with a single equipment examining people more than 10 times per day in central hospitals(43.19%)is higher than that in general hospitals(17.56%).The proportion of imaging equipment with less than 10 times in the same type of equipment is 73.08% for common X-ray machine,73.33% for CR,71.43% for DR,78.51% for black-and-white ultrasound and 64.00% for color Doppler ultrasound.4.Evaluation of the use effect of image equipment and analysis of its influencing factors: evaluation of the use effect of image equipment in township health centers from 5 dimensions of clinical effect,operation and operation,safety service,maintenance and after-sales training,and 11 evaluation indexes.25.34%,63.02% and 11.64% of the image equipment were "well received","medium" and "poor".Among all kinds of equipment,Dr was the highest(100.00%),followed by color ultrasound(43.33%),ordinary X-ray machine was the highest(28.56%),followed by black and white ultrasound(12.00%).The analysis results of the influencing factors of the use effect of image equipment in township health centers showed that the unit level,the promotion of practice qualification,the rise of education background were positively correlated with the total score of use effect,while the male was negatively correlated with the total score of use,and the influencing factors of the use effect of X-ray equipment were the unit level,the promotion of practice qualification,the satisfaction of radiation protection conditions,the increase of the number of daily examiners and the use effect There was a positive correlation between the total score of the results and the influencing factors of the use effect of ultrasound diagnostic instrument.5.The allocation of imaging health personnel: 79.25% were under 36 years old,4.40% were over 46 years old,and 28.31% worked for less than 5 years.The majority of the staff were low academic qualifications,low professional titles and low professional qualifications,54.72% were junior college education,32.07% were technical secondary school or below,89.31% were junior junior,60.38% were assistant medical practitioners,71.70% were staffed,76.10% were graduates of imaging major,and the time for further training was 160.15±121.90 days.The average annual income of imaging health workers is 402±55 yuan,34.82% of radiation workers are not satisfied with the radiation protection conditions,73.21% of radiation workers cannot normally enjoy radiation leave,and the average monthly radiation allowance is 63.99 ± 39.01 yuan,only 8.93% of township health centers have reached the standard.In the past three years,46 people left 56 township health centers,56 people were recruited and imported,and 50.00% of the former left because of the low treatment in township health centers.Conclusions:1.The allocation of imaging equipment in township health centers in Ganzhou City is not balanced.The number and grade of existing imaging equipment in central hospitals are better than those in general hospitals.There is still a certain gap between township health centers and the X-ray equipment allocation standard(DR)stipulated by Jiangxi Province in 2014.2.The phenomenon of overdue use of imaging equipment in township health centers in Ganzhou City is obvious,especially in general hospitals.The replacement of imaging equipment in central hospitals focuses on X-ray equipment,and DR;is the first choice.CR is the first choice for replacing X-ray equipment in general hospitals.The common use efficiency of imaging equipment in township health centers is not high.3.The existing imaging equipment in Ganzhou township health centers is mainly domestic equipment(except CT).Domestic imaging equipment has a large development space and market in township health centers.4.Unit level,professional qualification and educational background are the main factors that affect the use effect of imaging equipment.The improvement of unit level and practice qualification,satisfactory radiation protection conditions and increased number of daily examiners can improve the evaluation of the use effect of X-ray equipment,while the improvement of unit level and practice qualification and color Doppler ultrasound can improve the evaluation of the use effect of ultrasonic diagnostic instruments.5.Ganzhou township hospital image medical health human resource structure is unreasonable,such as the employees income is low,the mobility of personnel is large and it lack of a good environment for business ability promotion;the personnel tend to be younger,mainly with low education,low professional title,low professional qualification,according to a certain proportion of unqualified personnel,there are security risks and medical risks in the diagnosis and treatment activities.6.The use efficiency of imaging equipment in township health centers in Ganzhou is generally not high.The imaging equipment of different grades should be gradually replaced according to the service needs and service capabilities of the hospital.
