[Objective]1.To study the reliability of A simple method for detecting the titer of IgG anti-a(B)antibody in type O women,and to evaluate the application value of this method in clinical detection.2.To investigate the titer distribution of IgG anti-a(B)antibody in type O pregnant women in nanchang,jiangxi.3.The study of ABO type O newborns-HDN(ABO newborns hemolytic disease,ABO-haemolytic diseases of the newborn)and mother of the correlation value,to predict the occurrence of neonatal ABO-HDN,timely cure a carefully monitored timely provide theoretical basis.[Methods]1.People use simple method and the traditional classic anti globulin experiment method(standard method)(hereinafter referred to as the traditional method)for detection in May 2018-December 2018 to test the antenatal examination or pregnancy in our hospital 277 cases of resistance against A or B type O women IgG antibody titer(inspection A103),anti B105)resistance,resistance against A + B69 example,A total of testing resistance against A172 cases B174 cases).Age 16-45 years old,healthy,no liver,kidney and blood system disease,irregular antibody negative.The husbands were all non-o(A/B/AB).The results of the simple method and the traditional classical anti-human globulin method were measured on the basis of the dilution ratio.Spearman correlation was used to analyze the correlation of IgG anti A(B)titer and make regression analysis.To evaluate the application value of this method in clinical detection.2.277 O-type women were followed up to trace their pregnancy outcomes and the occurrence of blood type,bilirubin and abo-hdn disease in the newborn.Blood type tests were performed on 145 newborns who were followed up.Three hemolysis tests(direct anti-human globulin test,dissociation test and dispersion test)and bilirubin test were carried out on the neonates detected with non-o type.After sorting out the data of maternal antibody titer and neonatal hemolysis,a database was established,and the grade data were first transformed into variables.The ROC curve of traditional and simple methods was drawn with newborn diagnosis outcome as the gold standard.3.Retrospective statistical analysis was performed on the distribution of IgG anti-a(B)titer in the late gestational period of o-type pregnant women in nanchang,jiangxi,and the distribution of IgG anti-a(B)titer in the late gestational period of au-hdn neonates diagnosed in nanchang,jiangxi.IgG-A(B)titer test was carried out on 1444 healthy women with type O who were hospitalized in our hospital from January 2019 to March 2020.Age 16-45 years old,healthy,no liver,kidney and blood system disease,irregular antibody negative.The husbands were all non-o.Three hemolysis tests and bilirubin tests were carried out on the non-O-type neonates.The maternal antibody titer and neonatal hemolysis data were collected,and the distribution histogram was plotted and analyzed.[Results]1.With the reciprocal titer of 64 as the cut-off point,the titer positive rate of IgG anti-a antibody in type O women was 62.79%(108/172)and 66.86%(115/172),respectively.The positive rates of parallel detection of anti-b antibody titer were 47.12%(82/174)and 62.07%(108/174),respectively.2=2.0,P > 0.05,no significant difference.The simple method and the traditional method were used to detect the IgG anti-a(B)titer in women with type O in parallel,and the result was consistent with 65.90%(228/346),and the probability of difference within one dilution degree was 91.9%(318/346).2.With the reciprocal of titer as the cut-off point of 64,the area AUC under the ROC curve of detecting IgG anti-a or anti-b antibody titer in the mother of type O to predict the occurrence of abo-hdn in the newborns of type O(type A,B,AB)was 0.899,the z value was 7.867,and the P < 0.05 was statistically significant.The AUC under the ROC curve was 0.790 AUC,and the z value was 3.965,P < 0.05,indicating statistical significance.3.The distribution of IgG anti A or anti B titer in women with O blood in the third trimester was 32 and below,64,128 and 256 were more common.As the reciprocal value of titer increases,the distribution frequency decreases,and 512 and higher values are less common.However,the distribution of IgG anti-a(B)titer in the late gestational period of the o-type mothers diagnosed with ABO-HDN neonates was changed,and the probability of the inverse titer being 512 was significantly increased in the regions of 64,128 and 256.4.With reciprocal value ?64 as the positive judgment,the correlation between ABO hemolysis and positive maternal antibody value in the diagnosis of A/B/AB neonates was 90.9%(100/110),and the false negative rate of maternal antibody titer test was 9.1%(10/110).[Conclusion]1.The simple method is reliable for the detection of IgG anti-a(B)titer in women with type O,which is consistent with the detection results of the traditional and classic anti-human globulin method.The simple method is simple,fast,inexpensive and suitable for mass detection.Both the traditional method and the simple method are adopted in the clinical detection work,which is simple and convenient.The results of the two methods can be verified by each other to improve the accuracy of the detection results.2.The simple method and the traditional standard method were used to detect the IgG anti-a(B)titer of type O women,and the cut-off point of the reciprocal titer of 64 was taken as the positive value.The effect value of type O mothers could better predict the occurrence of abo-hdn disease in their non-type O children.For newborns of type A,B and AB with A maternal value of 1:64 or above,they should be closely followed.To monitor the occurrence of hemolysis and jaundice,the patients with severe hemolysis should be timely treated with albumin injection,blue light,and even blood exchange,so as to reduce the occurrence of neonatal bilirubin encephalopathy and even death.3.The distribution range of IgG anti-a(B)titer in late-gestational stage of o-type women in nanchang,jiangxi was different from that in late-gestational stage of au-hdn neonates.IgG anti-A(B)titer was more widely distributed in the third trimestery of O mothers diagnosed with ABO-HDN neonates.Gestational non-O-type(A/B/AB)fetuses may increase the maternal IgG antibody titer.4.Maternal antibody potency was associated with abo-hdn in neonates or fetuses of non-o(A/B/AB).
