Objective:To investigate the dynamic changes and clinical significance of serum CRP/ALB ratio in patients with acute pancreatitis(AP)at different time periods within 7 days after admission.Methods:The medical records of 120 patients with acute pancreatitis admitted to the emergency ICU,comprehensive ICU and gastroenterology department of the second affiliated hospital of nanchang university from January 2019 to June 2019 were retrospectively analyzed.According to the improved Marshall score>2,they were divided into severe(SAP)group and mild(MAP)group.Compare the two groups of patients hospitalized and 1,3,5,7 days after admission of CRP/propagated ratio,CRP,delta propagated,APACHE-? score difference;Is all the object of study on admission,1,3,5,7 days after admission CRP/propagated ratio of SAP's assessment of the value and CRP,delta propagated,APACHE-? score to the difference of the value of the evaluation of SAP;The correlation between CRP/ALB ratio and Marshall score in SAP group at admission and at day 1,3,5 and 7 after admission was analyzed dynamically.Results(1)on admission,1,3,5,7 days after admission SAP group of CRP,delta propagated,CRP/propagated ratio,APACHE-? scores were significantly higher than that of MAP group(P<0.05);(2)on patients with CRP/AP ratio propagated the area under the curve(AUC)in the diagnosis of SAP was 0.974,and significantly higher CRP,delta propagated,APACHE-? grading diagnosis AUC values of SAP(P<0.05).(3)1,3,5,7 days after admission of CRP/AP patients diagnosis of SAP AUC propagated ratio were 0.955,0.975,0.906,0.923,of which 1,3,5,7 days after admission AP CRP/propagated ratio in the diagnosis of patients with SAP the AUC value is significantly higher than CRP,APACHE-? score the AUC value(P<0.05),1,3 patients with AP CRP/propagated after admission ratio in the diagnosis of SAP,the AUC value is significantly higher than delta propagated the AUC value,However,there was no significant difference between the two on the 5th and 7th days after admission(P>,0.05).(4)the correlation coefficients(r)between CRP/ALB ratio and Marshall score of patients in SAP group at admission and day 1,3,5 and 7 after admission were 0.883(P<0.05),0.855(P<0.05),0.883(P<0.05),0.807(P<0.05),and 0.807(P<0.05),suggesting that CRP/ALB ratio in SAP group was strongly correlated with Marshall score within 7 days after admission,and was positively correlated with Marshall score(r>,0).Conclusion:(1)on patients with CRP/AP ratio propagated the illness severity of AP has good evaluation value,and its prediction of SAP role than CRP,delta propagated and APACHE-? score of prediction function in SAP,CRP/propagated ratio of high value indicates the AP patients degree is serious.(2)the CRP/ALB ratio of AP patients within 7 days after admission still has a good continuous evaluation value for the severity of AP.However,with the extension of time,it had no advantage over ? ALB's evaluation value for AP.(3)the CRP/ALB ratio of SAP patients within 7 days after admission has a good continuous correlation with Marshall score.To some extent,the CRP/ALB ratio within 7 days after admission can evaluate the degree of organ failure of SAP patients.
