Study On The Current Situation Of Health Education And Intervention Of STD Outpatients Based On Empowerment Theory

Objective:1.Based on the current situation of health education of STD outpatients,the intervention based on empowerment theory was established.2.To explore the influence of the intervention based on the empowerment theory on the medical behavior,related behavior,STD service,satisfaction,STD basic knowledge,anxiety and depression of STD outpatients by stimulating their subjective initiative,so as to provide an effective way to explore the health education intervention for STD outpatients.Method:1.Investigation on the current situation of health education of STD outpatients.From April to October 2018,1494 eligible STD outpatients from 7 hospitals in different districts of Shanghai were enrolled.2.Health education intervention of STD outpatient based on the empowerment theory.230 STD outpatients were enrolled in the study.First,experimental group were intervened based on the empowerment theory after the initial diagnosis,diagnosis,follow-up and three-month follow-up.Control group were intervened with the existing health education methods.At the end of the intervention,the differences of medical behaviors and related behaviors,good services,satisfaction,basic knowledge,anxiety and depression of the patients between the two groups were analyzed by chi square test and repeated measurement analysis of variance.Result:1.Baseline survey results show that:(1)Medical behaviors of STD patients: the time from the initial suspicion to the first visit was 40.97 ± 89.137 days;10.51% of the patients had been to private hospitals or private clinics for STD diagnosis and treatment;53.61% of the patients did not know how they were infected;only 28.05% of the patients consulted medical professionals after the onset of the disease.(2)Sexual behaviors and related behaviors of STD patients: if unsafe sexual behaviors occured,only 42.03% of them would actively seek for STD counseling and testing services;27.04% of them are homosexuality or double sexuality;14.32% of them had extramarital sexual behaviors;21.95% still had sexual behaviors after suspected infection of STD,of which only 58.54% used condoms;34.07% had been tested for AIDS.(3)39.49% of them knew the basic knowledge of venereal disease,39.29% of them had anxiety and 44.58% of them had depression.(4)The way of STD / AIDS health education that respondents received in the last three months: the most important way is by TV programs;60.71% of patients urgently need one-to-one consultation.2.Intervention based on empowerment theory:117 cases in the experimental group and 113 cases in the control group were enrolled.(1)After the intervention based on empowerment theory,the rate of notification of sexual partners,the rate of both treatment,the rate of condom use in sexual behavior,the incidence of high risk behaviors,and the rate of active detection of AIDS in the patients were better than those in the control group(P < 0.05).(2)After intervention,the mastery of STD related knowledge in the experimental group was better than that in the control group,and the scores of anxiety and depression were lower than those in the control group(P < 0.05).(3)After intervention,the service rate of six indicators in the experimental group was higher than that in the control group(P < 0.05).(4)After intervention,the patients in the experimental group were more satisfied with the health education and the overall evaluation of the satisfaction with the health education in STD clinic than those in the control group(P < 0.05).Conclusion:1.Baseline survey showed that STD patients had weak self-protection awareness,high incidence of depression and anxiety,and poor mastery of STD prevention and control knowledge,especially in the correct use of condoms,prevention of high-risk behaviors and medical treatment.Therefore,it is necessary to carry out publicity and education for STD patients.2.Health education guided by empowerment theory can stimulate patients' subjective initiative,improve patients' awareness rate of relevant knowledge,treatment compliance,reduce recurrence rate and improve patients' psychological status in the process of health education.Based on the theory of empowerment,the implementation scheme of health education intervention for STD outpatients has better practical value.
