| Objective: To study the clinical application of general anesthesia combined with intermuscular brachial plexus block in the treatment of giant rotator cuff under shoulder arthroscopy. Methods : RandomLy selected from our hospital in December 2018-2019,elective in shoulder arthroscopy Forty patients with massive rotator cuff repair were divided into experimental group and control group according to the random number table method. The experimental group was treated with general anesthesia combined with brachial plexus block(0.4% ropivacaine 15 mL);Only general anesthesia was performed for control group. The blood pressure, heart rate,BIS and other related indicators were changed during the operation, incision, intubation, extubation and 15 min interval. The anxiety scores of the two groups were changed.Opioid consumption, recovery time, satisfaction, postoperative pressure of electronic pump and VAS scores of 30 min,6 h,and 24 h after surgery were compared and analyzed. Results: after comparing the two groups, experimental group patients heart rate, mean arterial pressure in intraoperative 65 min,75 min,90 min were significantly lower, difference between groups was statistically significant(P < 0.05),electronic postoperative analgesia pump pressure, awakening time reduced significantly, postoperative day 1 anxiety levels decreased significantly, the difference between groups was statistically significant(P <0.05).There was no significant difference in patient satisfaction between the two groups. Conclusion: general anesthesia combined with intermuscular sulcus brachial plexus block is more suitable for the repair of giant rotator cuff under shoulder arthroscope.Because the intermuscular sulcus brachial plexus block can provide effective analgesia and muscle relaxation,it can reduce the surgical emergency, make the surgical field clearer, reduce intraoperative bleeding, and accelerate the postoperative functional recovery of patients. |
