| ObjectiveIn this study,by observing the difference between the anxiety and depression of menopausal women on the preoperative and postoperative clinical results of arthroscopic rotator cuff repair patients,this study explored the correlation between the anxiety and depression of menopausal women and the prognosis of arthroscopic rotator cuff repair.Factors related to the prognosis of arthroscopic rotator cuff repair..It provides a new theoretical basis for the physical and psychological treatment of patients undergoing arthroscopic rotator cuff repair surgery.MethodsProspective collection of female patients in the General Hospital of Guangdong Provincial Hospital of Traditional Chinese Medicine who underwent arthroscopic rotator cuff repair and menopause from June 2020 to January 2021,and selected 72 patients who met the inclusion and exclusion criteria and had complete data.Collect baseline data of patients within 48 hours of admission and parallel questionnaires,including self-rating depression scale(SDS),self-rating anxiety scale(SAS),visual analogue scale(visual analogue)scale,VAS)score,American Shoulder and Elbow Surgeons’ Scale(ASES)and Simple Shoulder Test(SST),and the patients were evaluated for 1 month after surgery,Follow-up for 3 months and 6 months of depression and anxiety(SDS,SAS scores)and clinical results(VAS,ASES,STT scores)of the patient’s surgical prognosis,enter the collected data into the Excel database,and use SPSS for the collected data 23.0 Statistical software for statistical analysis.By separately assessing the differences in preoperative and postoperative outcomes between groups with and without depression and with and without anxiety,to explore whether depression and anxiety have an impact on the prognosis of rotator cuff repair surgery,and analyze the correlation between multiple single factors and postoperative clinical outcomes After sex,a multiple linear regression model was established to explore the relevant factors affecting the prognosis of rotator cuff repair.Results1.A total of 72 patients were enrolled in this study.7 cases were lost to follow-up during the follow-up period,and 65 patients were normally enrolled.They were followed up for at least 3 months,of which 27(42%)were followed up for 6 months with an average follow-up time of(4.88± 1.33)months and an average age of(52.43±4.26)years.Among 65 patients,17 patients(26.2%)had depression tendency:13 patients(20%)were mildly depressed,4 patients(6.2%)were moderately depressed;15 of 65 patients(23.1%)had patients Anxiety tendency:13 patients(20%)with mild anxiety,2 patients(3.1%)with moderate anxiety,and 39 patients without depression or anxiety.2.The preoperative clinical results(VAS score,ASES score,STT score)and the clinical results(VAS score,ASES score)of the two groups with depressive tendency and non-depressive tendency were statistically significant(P<0.05),there was a statistically significant difference in ASES scores between the two groups with and without anxiety tendency only at 3 months after surgery.Regardless of whether there was depression or anxiety before surgery,the clinical results(VAS score,ASES score,STT score)at 3 months after surgery were significantly different from those before surgery(P<0.001).The clinical and psychological results at 6 months after the operation were significantly different from those before the operation(P<0.001).The results of univariate correlation and multiple linear regression analysis showed that the preoperative SDS score(P=0.046)was a prognostic factor affecting the VAS score at 3 months after surgery;age(P=0.008),tear size(P=0.007),Preoperative SDS score(P<0.001)and preoperative SAS score(P=0.025)are the prognostic factors that affect the ASES score 3 months after surgery,and the size of the tear(P=0.016)affects the STT score 3 months after surgery Prognostic factors.ConclusionAt present,female patients undergoing arthroscopic rotator cuff repair and in menopause have a higher incidence of depression and anxiety.Preoperative depression and anxiety have a negative impact on pain and function before and after rotator cuff repair.Arthroscopic rotator cuff repair can effectively improve the physical and psychological symptoms of patients.Patient age,rotator cuff tear size,preoperative depression,and anxiety scores are unfavorable factors affecting the prognosis of rotator cuff repair,and can be used as predictive tools for predicting postoperative pain relief and functional recovery.Therefore,in addition to paying attention to the physical symptoms of patients,clinicians should also pay enough attention to the psychological symptoms of patients. |
