Diabetes is another critical disease that destroys human health and life followed tumors,cardiovascular and cerebrovascular diseases.Nearly a quarter of diabetic patients will have complications such as delayed wound healing,diabetic ulcers,and diabetic foot,which are major causes of death or disability in diabetic patients.The pathogenesis of chronic wound in diabetes including prolonged wound inflammation,neurological disorders,multiple immune cell dysfunction,etc.In addition,it is one of the most important pathogenesis for a long inflammatory period to hinder healing process.There is yet no suitable solution for this pathology.Therefore,it is one of the feasible solutions to design a dressing that can alleviate the excessive inflammatory reaction of the wound to promote the repair of diabetic wound.Acetylated konjac glucomannan(Ace KGM)is able to promoting wound healing as it can shorten the inflammatory period,stimulate the transformation of macrophages to the antiinflammatory phenotype(M2)and make them secrete the pro-healing factors.Collagen(COL)and hyaluronic acid(HA)are important extracellular matrix components in skin tissues,which can effectively promote wound healing;and sponge structure can absorb wound exudate,which is conducive to wound cell growth and adsorption.To sum up,this study intends to design Ace KGM / COL / HA composite sponge for wounds repair in diabetic SD rats,expected to obtain a dressing that promotes the repair of diabetic wounds.Konjac glucomannan(KGM)was degraded with a molecular weight of 8,23 and 56 k Da(named KGM1,KGM2,KGM3)by sulfuric acid degradation method,and then acetylated by acetic anhydride.Add different proportions(10%,20%,30%,40%)of Ace KGM to a mixed solution of COL and HA(COL: HA = 9: 1),freeze-dry to form Ace KGM / COL / HA compound sponge.The physical and chemical characterization of composite sponges,cells' cytocompatibility,the regulation on macrophages,and the effects on the healing of full cortical defects in SD diabetic rats,were evaluated.The results are as follows:The results showed that the COL / HA composite sponge containing 20% Ace KGM had good spatial structure and stability.The cell experiments disclosed that the Ace KGM / COL / HA composite sponge was non-cytotoxic,and could promote the transformation of macrophages into M2 and promote the secretion of IL-10.Among them,the Ace KGM2 / COL / HA composite sponge was the best one.Animal experiments proved that the Ace KGM / COL / HA composite sponge could promote the growth and repair of diabetic wound tissues,and the Ace KGM2 / COL / HA group was more effective.These demonstrates that the Ace KGM / COL / HA composite sponge has the potential to be an alternative material for the healing of diabetic chronic wounds.
