Part ? Intravoxel Incoherent Motion Diffusion Weighted Imaging Combined With Texture Analysis Baesd on T2 WI in Diagnosis of Cervical Cancer With Parametrium InvasionObjective To investigate the value of intravoxel incoherent motion diffusion weighted imaging(IVIM-DWI)and texture analysis in the diagnosis of cervical cance-r with parametrium invasion.Method From January 2015 to July 2018,83 patients with endometrial carcinoma confirmed by pathology were retrospectively analyzed.They were divided into low grade(including grade ?/?)group and high grade(grade ?)group,56 cases in low grade group(16 cases in grade ?,40 cases in grade ?)and 27 cases in high grade group(grade ?).GE Analysis Kit(AK)software was used to map the lesion ROI manually layer by layer in delayed MRI enhancement,and then the global gray histogram parameters of the lesion were obtained.After data preprocessing,random allocation of training group andverification group according to training group data ratio of 0.69.The Lasso method was used to screen the characteristic parameters,SVM was used toconstruct the prediction model,and ROC of the verification set data was used to evaluate the effectiveness of the model.Results There were 57 cases in training group(39 cases in low grade,18 cases in high grade)and 26 cases in verification group(17 cases in low grade and 9 cases in high grade).The AK software extracted 42 gray-scale histogram characteristic parameters of the lesions in the whole region,of which 13 parameters were statistically different among the high and low level groups.The characteristic parameters after screening by Lasso are Range and uniformity.Based on the above two histogram parameters,the prediction model of endometrial carcinoma grade was constructed by SVM.The accuracy,specificity and sensitivity of the training set and the test set were 0.75,0.85,0.56 and 0.85,0.94,0.67,respectively.The area under the ROC curve of the validation set is 0.81.Conclusion Based on the features of MRI histogram,it is valuable to predicthigh and low grade endometrial carcinoma before operation,and can providemore imaging biomarkers for preoperative grading assessment of endometrialcancer.Part ? Value of Intravoxel Incoherent Motion Diffusion Weighted Imaging in Evaluating Different Histological Type of Cervical CancerObjective To explore the value of IVIM-DWI in distinguishing squamous cell carcinoma of cervix from adenocarcinoma and small cell neuroendocrine carcinoma.Method The clinical and imaging data of 51 patients with pathologically confirmed cervical cancer were analyzed retrospectively.They were divided into three groups according to different pathology results,the 40 cases of squamous cell carcinoma of cervix,the 7 cases of adenocarcinoma and the 4 cases of small cell neuroendocrine carcinoma.Conventional magnetic resonance image and IVIM-DWI were performed preoperation.The ROI file which was imported into the post-processing software of GE company was applied to draw ROI manually in three consecutive layers on the IVIM-DWI parameters(ADC,D,D*,f).The SPSS 23.0 software's one-way ANOVA analysis,LSD-t test and Kruskal-Wallis test were used to compare the data of three groups,then analysed whether there was statistical difference among the three groups of IVIM-DWI parameters.Besides,the AUC is adopted to evaluate the diagnostic efficiency.Results There was no significant difference(P>0.05)among the D* and f in the differential diagnosis of squamous cell carcinoma of cervix,adenocarcinoma and small cell neuroendocrine carcinoma whereas the difference was significant(P<0.05)among the ADC and D of the three groups,respectively.Besides the AUC of ADC,D of SCC and CAC group is 0.711?0.888;while those in SCC and SNEC is 0.988?0.913.Conclusion IVIM-DWI has a certain value in the differentiation of squamous cell carcinoma of cervix from adenocarcinoma and small cell neuroendocrine carcinoma,among which ADC and D value have higher diagnostic efficiency.
