Clinical Experience Of Director Yang Yulan In Differential Diagnosis And Treatment Of SLE With Anxiety And Depression Based On Data Mining

Objective: Taking the patients admitted to the second affiliated Hospital of Tianjin University of traditional Chinese Medicine as the research object,this paper makes a retrospective study on the case data and traditional Chinese medicine prescriptions of SLE patients in Director Yang Yulan’s outpatient clinic and Nephropathy and Rheumatology Department from September 1,2017 to January 23,2020 by using modern psychological measurement tools.Data mining summarizes their clinical experience in the diagnosis and treatment of SLE anxiety and depression.To provide a new method for inheriting Professor Yang Yulan’s academic ideas,and to provide experience for traditional Chinese medicine in the treatment of anxiety and depression in SLE.On this basis,based on a broader train of thought to guide the development of the theoretical system of traditional Chinese medicine,looking for new development and new ideas of traditional Chinesemedicine.Methods: Clinical research adopts the method of retrospective analysis,through face-to-face investigation and telephone consultation,follow-up of SLE patients in Director Yang Yulan’s outpatient clinic and Nephropathy and Rheumatology Department of the second affiliated Hospital of Tianjin University of traditional Chinese Medicine from September 1,2017 to January 23,2020,collect and sort out the patients’ clinical cases and prescriptions of traditional Chinese medicine,investigate and measure the subjects with the standard psychological scale,and fill in the psychological scale by themselves.Such as ZUNG anxiety self-rating scale,mental health self-rating scale,etc.,to obtain the anxiety disorder,depression disorder and mental health level of each SLE patient,and to collect basic information and related clinical information in the form of questionnaire data(self-filled in by the research object),combine correlation analysis and frequency statistics,construct a database through Excel,through frequency statistical analysis,Statistics were made on the syndrome type of treatment of SLE,the frequency of use of traditional Chinese medicine,the frequency of drug use and so on,and the law of drug use was comprehensively observed.Then use IBM SPSS Modeler24.0 to process the data with association rules Apriori algorithm to explore the potential relationship between tutors using traditional Chinese medicine and summarize their clinical experience in the diagnosis and treatment of anxiety and depressionin SLE.Result:Data mining showed that:a total of 48 SLE patients with anxiety and depression,289 medical records and prescriptions of traditional Chinese medicine were included,involving178 kinds of traditional Chinese medicine,with a total frequency of 3704 times.The drugs used more than 75 times are raw astragalus,Qingfeng vine,Artemisia annua,Ligustrum lucidum,raw licorice,Zelan,earth dragon,Hedyotis diffusa,raw Radix Paeoniae Alba,Radix Rehmanniae,Rhodiola,Poria cocos,fried white peony,turmeric,Polygala,Pueraria,Salvia miltiorrhiza,Angelica sinensis,Coptis chinensis,Bupleurum,Radix scrophulariae,Scutellaria baicalensis.These drugs are commonly used by Director Yang Yulan in clinical treatment of anxiety and depression in SLE.The top 10 traditional Chinese medicines are Radix Astragali,Rhizoma Qingfengteng,Artemisia annua,Ligustrum lucidum,raw licorice,Zelan,Dilong,Hedyotis diffusa,Radix Paeoniae Alba,Radix Rehmanniae,Rhodiola,Poria cocos,roasted Radix Paeoniae Alba,turmeric and Polygala,which are the characteristic drugs of Director Yang Yulan in the treatment of anxiety and depression in SLE.Among the commonly used drugs,there are 18 drug categories,which are ranked from large to small according to the frequency of use.they are deficiency medicine(tonic medicine),heat-clearing medicine,activating blood circulation and removing blood stasis medicine,relieving surface medicine,dispelling wind and dampness medicine,tranquilizing medicine,calming liver and extinguishing wind,regulating qi,promoting water and permeating dampness,astringent medicine,hemostatic medicine,expectorant,antitussive and asthma medicine,digestion medicine,dampness-relieving medicine,warming medicine,purgative,poison-killing and insect-killing anti-itching drug,and orifices-relievingdrug.The psychological characteristics of SLE patients: the scores of SCL-90 SLE patients were lower than the norm,the scores of depression and anxiety of SLE patients were higher than those of the norm,and the scores of extroversion and extroversion and emotion were higher than those of psychoticism and concealment relatively high.Among the four factors of sex,age,marriage and course of disease,marriage and age significantly affect mental health.For married,younger SLE patients,the level of mental health is relatively low.The two dimensions of anxiety and emotion have a great impact on the level of depression,forming a very obvious positivecorrelation.Emotional,phobic,sensitive and somatization factors have a great impact on the level of anxiety.Among them,there is a negative correlation between sensitive factors and anxiety level,and there is a positive correlation between other factors and anxiety level.Comparison of psychological dimensions of SLE patients of different TCM syndrome types:comparing SLE patients with liver and kidney yin deficiency syndrome with SLE patients with positive syndrome,there are more obvious psychological problems such as obsessive-compulsive,sensitive,depressed,anxious,hostile,paranoid,psychotic symptoms and so on.The SLE patients with deficiency of both heart and spleen syndrome have more obvious psychological disorders of hostility and obsessive-compulsive disorder compared with the SLE patients with syndrome of deficiency of both heart and spleen.Conclusion:1.Yang Yulan often uses raw astragalus,Qingfengteng,Artemisia annua,Ligustrum lucidum,Zelan,Dilong,Hedyotis diffusa,Radix Paeoniae Alba,Radix Rehmanniae,Rhodiola,Poria cocos,turmeric,Polygala and other drugs to treat anxiety and depression in patients with SLE.2.Yang Yulan,mentor,believes that the main pathogenesis of anxiety and depression in SLE is based on deficiency of liver and kidney,heat toxin and bloodstasis.3.To treat emotional diseases from the perspective of Zang-fu organs,we should take the adjustment of Yin and Yang of the five Zang organs as the starting point,attach importance to regulating Yin and Yang and establishing Zhongqi,and calm the mind and mind by reconciling Yin and Yang of the five Zangorgans.4.The main syndrome types of this disease are heat toxin blazing syndrome,yin deficiency heat syndrome,heart and spleen deficiency syndrome,qi and blood deficiency syndrome,according to different syndrome types,they are treated by tonifying the liver and kidney,tonifying qi and consolidating the foundation,clearing heat and nourishing yin,relieving depression and calming the mind,and so on.5.Chinese traditional medicine is rooted in the long splendid culture of China,and the thought of regulating mind is contained in it.TCM’s understanding of human health and disease is grasped from variousangles.6.SLE patients is poor.In the process of diagnosis,attention should be paid
