Objective To study the distribution characteristics of TCM syndrome elements in patients with perimenopausal central obesity,combine imaging with molecular biology and TCM theory,and analyze the correlation with adipokines,estrogen receptor and body fat,so as to provide micro indicators and reference for TCM diagnosis of perimenopausal central obesity.Methods1.According to the diagnostic criteria and inclusion criteria,the subjects were divided into two groups,namely the central group and the control group.2.The abdominal fat content,visceral fat content,subcutaneous fat content,BMI index,weight,waist circumference and height were detected by CT.3.Real time quantitative PCR detection system(q PCR)was used to detect the m RNA levels of estrogen receptor?and?,and enzyme-linked immunosorbent assay(ELISA)was used to detect the protein content of estrogen,leptin and visfatin in serum.Results1.Basic data: a total of 172 subjects were included in this study,including 100 cases of perimenopausal central obesity group and 72 cases of control group,with statistical difference in age between the two groups(P<0.01).2.Comparison of body fat parameters:(1)Visceral fat: central group > non central group,there was statistical difference between the two groups(P<0.01),but there was no significant difference in weight,BMI,waist circumference and subcutaneous fat between the two groups(P>0.05).(2)In perimenopausal central obesity group,there was a negative correlation between damp syndrome elements and visceral fat and subcutaneous fat,while there was a negative correlation between Qi deficiency syndrome elements and visceral fat.In the control group,there was no correlation between disease location syndrome elements and body fat parameters.3.Distribution of syndrome elements:(1)In perimenopausal central obesity group,phlegm(92.85%),dampness(89.68%),yin deficiency(80.15%),Qi deficiency(72.2%)and Qi Stagnation(70.63%)were the main pathogenic factors.The disease location syndrome elements were mainly liver(81.74%),kidney(78.57%),spleen(54.76%).(2)In the control group,phlegm(90.6%),dampness(88%),Qi Stagnation(70.66%),yin deficiency(64%)and heat(61.33%)were the main pathogenic factors in the control group.The disease location syndrome elements were liver(70.6%),kidney(49.33%),spleen(57.33%).(3)Comparison of syndrome element scores between the two groups: the phlegm dampness syndrome element score in the central group was higher than that in the control group(P>0.05),the yin deficiency syndrome element score in the central group was significantly higher than that in the control group(P<0.01),the liver syndrome element score in the central group was higher than that in the control group(P<0.05),and the spleen syndrome element score in the control group was higher than that in the central group(P<0.05).Conclusions1.The occurrence of perimenopausal central obesity is closely related to liver and kidney.2.There is correlation between Qi deficiency syndrome element and dampness syndrome element and visceral fat.3.There is a positive correlation between spleen syndrome elements and visfatin.In clinic,visfatin level can be regulated by restoring spleen function and reducing visceral fat content.4.There is a negative correlation between kidney syndrome elements and estrogen.It can also regulate hormone levels by nourishing the kidney to achieve weight loss and improve perimenopausal symptoms.ER?is closely related to the occurrence of abdominal obesity.5.Er ? and ER ? are closely related to abdominal obesity.
