Patterns Of Posttraumatic Stress Disorder And Posttraumatic Growth Among Breast Cancer Patients In China:A Latent Profile Analysis

Objective:There were widespread Posttraumatic Stress Symptom and Posttraumatic Growth in breast cancer patients.However,the pattern of inter-individual heterogeneity of traumatic response was unclear.The aim of this study was to investigate the current status of posttraumatic response,to explore the specific pattern and predictors of posttraumatic responses,and differences in anxiety and depression in different posttraumatic response patterns.Method:During the period from January 8,2018 to April 27,2018,a total of 612 breast cancer patients during chemotherapy were enrolled in a top three hospital in Nanjing,Jiangsu Province.A questionnaire survey method was used for all participants.The questionnaire package includes demographics and some clinical medical conditions,Posttraumatic Stress Disorder Symptom Scale,The Posttraumatic Growth Inventory,Network of Relationships Inventory,Generalized Anxiety Disorder,Patient Health Questionnaire-9.Modeling was performed using a Latent Profile Analysis to explore patterns of posttraumatic stress symptom and posttraumatic growth in breast cancer patients during chemotherapy.Result:Based on the fitting indicators of the latent profile analysis,three subtype models of posttraumatic response were the optimal model.(a)Resisting group(34.640%):Patients reported less both posttraumatic stress symptoms and posttraumatic growth.(b)Growth group(47.386%):Patients showed less posttraumatic stress symptoms and higher levels of posttraumatic growth.(c)Struggling group(17.974%):Patients showed more both posttraumatic stress symptoms and posttraumatic growth.Breast cancer patients with lower income were more likely to belong to the resisting group and the struggling group.Breast cancer patients with high levels of social support were more likely to belong to the growth group.Moreover,patients in the struggling group had the highest levels of anxiety and depression,followed by the resisting group.Patients in the growth group had the lowest levels of anxiety and depression.Conclusion:It was found that there was heterogeneity in the posttraumatic response pattern of breast cancer patients,which means there were there kinds of posttraumatic response patterns in patients during chemotherapy.The study didn't distinguish the pain group,which was consist with previous studies among other samples in China.Nearly half of breast cancer patients belong to the growth group,and most breast cancer patients were not in pain as imagined.Family income levels and social support could predict posttraumatic response patterns,and negative emotions in breast cancer patients differ in different posttraumatic response patterns.The results provided theoretical support for design about different scheme of clinical psychological interventions,suggesting that necessity of hierarchical and different ways of psychological care who afford by paramedics for breast cancer patients with different posttraumatic response patterns.
