Objective:Diabetes mellitus,as one of the three major chronic diseases,has a very high incidence rate,and studies have proved that the late stage of diabetes mellitus has a higher risk of suffering from muscular dystrophy syndrome.Some studies have shown that aerobic combined with anti-resistance exercise can improve the condition of diabetes and increase the exercise capacity,but few studies have been done on the muscle improvement effect of exercise on diabetic patients and the improvement effect on the risk of muscle reduction syndrome.This study intends to discuss the improvement of muscle quality,strength and shape of patients with type 2 diabetes and the changes of biochemical risks related to muscle reduction through 12-week aerobic combined resistance exercise,and observe the relationship between the two,so as to provide theoretical basis for guiding diabetes patients to prevent muscle reduction.Methods:Through general investigation,Exercise Cardiopulmonary Test and blood biochemical examination,20 patients with type 2 diabetes who met the requirements were screened.Subjects were randomly divided into control group(N=10)and exercise group(N=10).Subjects in the exercise group received 3 months of aerobic combined resistance exercise training.Aerobic exercise is performed by bicycle or elliptical machine,and resistance exercise is performed by elastic belt.The duration of exercise is 3 times/week,and the intensity of exercise is 60%VO2R or RPE=11-13(6-20).The duration of exercise is about 60-70 minutes,including 5-10 minutes of warm-up and finishing activities.The control group maintained its original lifestyle.The body composition,muscle mass,muscle strength,muscle morphology and biochemical indexes related to muscle reduction of the subjects were measured before and after the intervention.The data of the front and back sides of the control group and the exercise group were compared by paired sample T test.The baseline comparison,back side comparison and difference comparison of the control group and the exercise group were compared by independent sample T test.All data were processed and analyzed by SPSS 20 and set in all hypothesis tests,p<0.05,indicating that the difference is significant.Results:(1)Intra-group comparison before and after the exercise group showed significant differences in body weight,BMI,BFR,ASM,SMI and ASMI(P<0.05).After 12 weeks of intervention,the data between the control group and the exercise group showed no significant differences in body weight,SMI and RSMI,but there were significant differences in BMI and BFR,and ASM(P=0.077)showed an increasing trend.The difference between the control group and the exercise group was significant(P<0.05).(2)Intra-group comparison before and after intervention in the exercise group showed significant differences in flexor muscle thickness,flexor elbow muscle El,upper arm circumference,rectus femoris circumference,rectus femoris El and thigh circumference(P<0.05).Compared with the data of the control group and the exercise group after 12 weeks of intervention,only the thickness of flexor elbow muscle and rectus femoris muscle showed a significant difference(P=0.004),and no significant difference was found in other indicators.The difference between the control group and the exercise group was significant(P<0.05).(3)Intra-group comparison before and after intervention in the exercise group showed significant differences in grip strength,cs-30 and PEF(P<0.05),except no significant changes in PEF.When comparing the data between the control group and the exercise group after 12 weeks of intervention,there was a significant difference(P<0.05)except no significant change in PEF.Comparing the difference between the control group and the exercise group,the difference was significant except PEF(P<0.05).(4)Intra-group comparison before and after intervention in the exercise group showed that,except for hs-cpr,there was a decreasing trend(P=0.056),significant differences were found in il-6,igf-1 and MSTN(P<0.05).Comparing the data between the control group and the exercise group after 12 weeks of intervention,except for no significant change in MSTN,igf-1 showed a significant difference,il-6(P=0.056)and CPR(P=0.074)showed a decreasing trend.The difference between the control group and the exercise group was significant(P<0.05).Conclution:(1)Moderate intensity aerobic exercise combined with resistance exercise can improve body composition and increase muscle quality in patients with type 2 diabetes.(2)Moderate intensity aerobic combined resistance exercise can increase muscle thickness,reduce limb circumference,reduce skeletal muscle echo intensity and improve fat infiltration in patients with type 2 diabetes.(3)Moderate intensity aerobic combined resistance exercise can improve the muscle strength of upper and lower limb skeletal muscles in patients with type 2 diabetes,but has no significant effect on the muscle strength of respiratory muscles.(4)Moderate intensity aerobic exercise combined with resistance exercise can improve the biochemical factors related to muscle reduction in patients with type 2 diabetes.
