| OBJECTIVE: pemphigus(pemphigus)is a relatively rare fatal autoimmune disease,it can be observed flaccid blisters or bullae appear on the patient’s skin and(or)mucous membrane in clinic.Without the systemic use of hormone treatment of pemphigus,Some patients with pemphigus were threaten by water electrolyte disorder,severe infection.Most patients died of systemic failure.With hormone treatment,the survival rate was significantly increased,but the mortality rate was still high,the cause of death is usually due to long-term use of hormone which induced complications,People began to try other treatments to control pemphigus,such as application of immunosuppressive agents.In recent years,the injection of gamma globulin has gradually become a routine treatment.The diagnosis of pemphigus needs to consider its clinical manifestations,skin histopathology and immuno pathology.36 cases of hospitalized pemphigus in our hospital were retrospectively analyzed by clinical features,laboratory tests,and mild,moderate and severe treatment of pemphigus in order to summarize the clinical characteristics of pemphigus,thinking about how to better control the condition of pemphigus patients and improve their prognosis.METHODS: We enrolled a total of 36 patients with pemphigus in Dalian Dermatology Hospital from June 2011 to December 2016,and register status,age,starting site,underlying disease,infection,mucosal involvement,laboratory examination,histopathology,immunopathology,length of hospital stay,treatment and efficacy by list to summarize and analyze.RESULTS:There are 36 cases in this stduy,17 male patients,accounting for 47.22%,19 female patients,accounting for 52.78%.The average age of onset was 58.53 + 11.4 years,with an average length of stay 9.20 ± 1.0954 days.5 cases of mild patients,accounting for 13.89d%,26 cases of moderate patients,accounting for 72.2%,5 cases of severe patients,accounting for 13.89%.12 patients(33.33%)had mucosal involvement.15 patients with pemphigus had other systemic diseases(41.67%).13 patients(36.11%)appeared infection,and skin infection was the most common(9 cases,25%).Systemic glucocorticoid therapy is the main treatment and there was a significant difference in the amount of initial glucocorticoid and the maximum Glucocorticoid in patients with mild,moderate and severe(P<0.05).Mucosal involvement and mucosal uninvolvement of the initial amount of glucocorticoid and the maximum amount of glucocorticoid had significant difference(P<0.05),during the course of treatment can also be combined with immunosuppressive agents and we can apply Immunoglobulin treatment to severe,poor efficacy or hormone contraindications patients.CONCLUSION:1.Pemphigoid occurs in the elderly 2.Infection is the most common pemphigus complication,especially skin infections and oral fungal infections.3.Glucocorticoids are the main treatment of pemphigus,severity and mucosal damage is the important factor influencing the dosage of glucocorticoid. |
