Health care is of great concern to millions of people,and it is a major livelihood issue in China.In recent years,along with the social economic transition process,such as aging and urbanization,the basic health demand of residents is growing rapidly,showing diverse characteristics,and the supply side structural problems remain prominent,bring challenges to the establishment and improvement of the basic medical and health service system.It is becoming increasingly difficult for individual medical organizations to rely on their own resources to solve the problems of rising medical costs,increasing demand,poor quality of diagnosis and treatment and low efficiency.Medical alliance has become the only way for medical organizations.The construction of medical association information platform is an important work in the process of implementing the graded medical treatment system in China.Healh information exchange(HIE)is one of the core components of medical association information platform.It aims to serve the purpose of co-construction,sharing and linking of inter-regional and inter-organizational clinical and patient information.The role of HIE in improving the efficiency,quality and accessibility of medical services and reducing the costs and risks of medical services has been fully validated and is recognized as the future development direction of health information construction.But to some extent,the role of HIE is still uncertain,mainly because of the inter-organizational HIE is influenced by many factors,such as organizational external environmental pressures and inter-organizational exchange relationships.Therefore,to explore the influence of these factors on medical organizations' willingness to use HIE will help to speed up the realization of grading diagnosis and treatment goals in China.HIE interacts with the structure of the medical industry,environment and social factors,and its use is influenced by the external organizational institutional environment and inter-organizational exchange relationship.In view of this,this article takes social exchange theory and institutional theory as the research angle to construct the medical association information platform health information exchange influence factor model.A total of 135 valid samples were collected by means of electronic questionnaire,and the partial least squares(PLS)structural equation model was used for empirical study.Based on effective sample data,the descriptive statistics analysis of samples,the reliability,validity and path test analysis of research model,inter-variable effect analysis and mediating effects of variables was carried out by means of SPSS19.0 and SmartPLS(v.3.2.6)tools.The empirical results indicate that the degree of reciprocity and common power or influence between medical organizations has a direct and significant influence on trust among medical organizations;the degree of reciprocity and trust among medical organizations has a direct and significant impact on the willingness of medical organizations to use HIE;the common power or influence of medical organizations has no significant impact on the willingness of medical organizations to use HIE;coercive pressure and mimetic pressure in institutional pressure have a direct and significant influence on the willingness of medical organizations to use HIE,and normative pressure has no significant influence.In addition,trust plays a complete mediating effect between common power or influence and HIE use intention;trust has a partial mediating effect between reciprocity and HIE use intention.
