Objective:To explore the optimal intervention time of ischemic postconditioning based on bibliometrics.Based on the results of the literature,the rats of myocardial ischemiareperfusion injury were treated with acupuncture at three different time points to study the optimal acupuncture time and the mechanism of action.Methods:Literature Study:A computer-based search of China National Knowledge Infrastructure(CNKI),Wanfang Database,Chinese Science and Technology Journals Database(VIP),Chinese Biomedical Abstract Database(CBM)and PubMed was conducted to collect modern literature about post-conditioning against myocardial ischemia-reperfusion injury,and the statistical analysis was adopted to analyze the year of publication,subjects,sample size,intervention measures,outcome indexes and experimental results of included literatures.The results of the literature study provided references for the set of acupuncture time.Experimental Study:There were 6 groups in the study,including acupuncture before reperfusion group,acupuncture with reperfusion group,acupuncture after reperfusion group,blank group,ischemia-reperfusion group,and non acupoint group.First 3 groups were set based on the results of the literature study.The model of myocardial ischemia reperfusion was established by ligating the left anterior descending branch of coronary artery for 40 mins and then reperfusion for 120 mins.Electrophysiological recording instrument and Tunel techniques were used to detect ECG and the rate of myocardial apoptosis in order to explore the optimal time of acupuncture post-conditioning for myocardial ischemia-reperfusion injury.qPCR and Western blot were used to detect the expression of A1 ADR and ?-OPR of mRNA and protein in myocardium to explore the mechanism of acupuncture postconditioning for myocardial ischemia reperfusion injury.Result:Literature Study:A total of 18 articles(14 Chinese articles and 4 English articles)were included in the study,including 2 clinical studies and 16 animal studies.The optimal treatment time for typical ischemic post-conditioning was early in reperfusion.The best time window for pharmacological post-conditioning depends on t.The optimal treatment time for distal ischemic post-conditioning was early in reperfusion.Experimental Study:1.ST segment changes:Compared with the normal ST segment,the ST segment was significantly elevated after ligation(P<0.05).Compared with ST after ligation,the ST segment was significantly reduced after 1 hour of reperfusion(P<0.05).2.The average apoptosis rate of cardiomyocytes: Compared with blank group,the average apoptosis rate of ischemia-reperfusion group,acupuncture before reperfusion group,acupuncture with reperfusion group,acupuncture after reperfusion group,and sham points group were higher(P<0.05).Compared with the ischemia reperfusion group,the average apoptosis rate of acupuncture before reperfusion group,acupuncture with reperfusion group,and acupuncture after reperfusion group were lower(P <0.05).Compared with the acupuncture with reperfusion group,the average apoptosis rate of sham points group were higher(P<0.05).3.A1 ADR mRNA expression: Compared with blank group,the expression of A1 ADR mRNA of ischemia reperfusion group was lower(P<0.05).Compared with the ischemia reperfusion group,the expression of A1 ADR m RNA of acupuncture with reperfusion group and acupuncture before reperfusion group were higher(P <0.05).Compared with the acupuncture with reperfusion group,the expression of A1 ADR mRNA of acupuncture after reperfusion group was lower(P < 0.05).Compared with the acupuncture before reperfusion group,the expression of A1 ADR mRNA of acupuncture after reperfusion group was lower(P<0.05).4.?-OPR mRNA expression: the expression of A1 ADR mRNA of 6 groups have no significant difference.5.A1 ADR protein expression: Compared with the ischemia reperfusion group,the expression of A1 ADR protein of acupuncture with reperfusion group and acupuncture after reperfusion group were higher(P<0.05).Compared with the acupuncture with reperfusion group,the expression of A1 ADR protein of acupuncture before reperfusion group and sham points group were lower(P<0.05).6.?-OPR protein expression: Compared with blank group,the expression of ?-OPR protein of ischemia reperfusion group was lower(P<0.05).Compared with the ischemia reperfusion group,the expression of ?-OPR protein of acupuncture with reperfusion group and acupuncture after reperfusion group were higher(P<0.05).Compared with the acupuncture with reperfusion group,the expression of ?-OPR protein of acupuncture before reperfusion group and sham points group were lower(P<0.05).Conclusion:1.Acupuncture post-conditioning can effectively reduce the apoptosis rate of myocardial cells,and protect impaired myocardium.2.In the 20 minutes before reperfusion,reperfusion immediately opened,reperfusion after the opening of the 20 minutes of the acupuncture can protect the myocardium.The optimal time point is opened immediately after reperfusion.3.The protection mechanism of acupuncture post-conditioning for impaired myocardium may be the activation of A1 ADR and ?-OPR expression.
