Concealed Incision Repairs Adult Recessive Cleft Lip

Objective:To discuss the method of correction for adults' microform cleft lip.Methods:Repair adults' microform cleft lip through covert incisions.20 adults' unilateral microform cleft lips were repaired by this method.The disrupted orbicularis oris muscle is exposed by oral mucosal incision to repair the muscle,and to reconstruct the nasolabial muscle tension line.Based on the muscle tension line group theory,firstly,the nasolabial muscle around the nasal floor(the first auxiliary tension line group)is reconstructed,and then the orbicularis oris muscle around the philtrum(the second auxiliary tension line group)is reconstructed.The deviation of the nasal septum is corrected through nasal mucosal incision.The abnormality of labial arch is corrected by Z-plasty.The nasal alar collapse and narial dissymmetry are corrected through Tajima incision of affected nasal alar.Results:The 20 postoperative patients are well improved.The patients are followed up for 6 months,and 18 of them get stable results with the improved appearance of nasal alar,nasal sill,nasal columella,Cupid bow and philtrum.2 of them suffer transposition of nasal alar and narial dissymmetry.Conclusions:Based on the muscle tension line group theory,using different covert incisions of oral mucosa,nasal mucosa and affected nasal alar can reconstruct the nasolabial muscle tension line to repair adults' unilateral microform cleft lip.This technique offers the ability to effectively form a good nasal alar,nasal sill,nasal columella,Cupid bow and philtrum.
