[Objective] : Perform studies on the anatomic structure of normal nasolabial portion and the pathological mechanism of secondary deformity of unilateral cleft lip. Repair the secondary deformity of unilateral cleft lip in the aspect of anatomic and physiological function, especially paying attention to the reconstruction of nasolabial subunit. Consequently, appropriate individualized therapeutic schemes are designed for the patients with secondary deformity of unilateral cleft lip.[Methods] : Individualized therapeutic schemes were designed for the patients with secondary deformity of unilateral cleft lip according to the principles of functional repair of orbicularis oris muscle, anatomic reduction of alar cartilage and reconstruction of nasolabial subunit. From 2003, 96 patients with secondary deformity of unilateral cleft lip were operated. The patients were followed up in the ways of telephone, mailing photos or returning hospital. The follow-up time is from 3 months to 5 years. Patient's and doctor's satisfaction survey were performed meanwhile.[Results] : In all the 96 patients, 10 were lost and 86 were followed up, in which 3 by mailing photos,9 by returning hospital and the others by telephone. As to the patient's satisfaction survey,81 patients were satisfied,l were unchanged and 4 were unsatisfied because of the obvious labial incision scar in 4 cases and asymmetric nostrils in 1 case; the satisfaction rate was 94.2%. As to the doctor's satisfaction survey on 12 patients with photos,ll were satisfied, and the satisfaction rate was 91.7%.[Conclusions] :The key point to repair secondary deformity of unilateral cleft lip is the fine anatomic disconnection, precise reduction and accurate involution of orbicularis oris muscle. The study on nasolabial vascular architecture mode is helpful to ensure nasolabial blood supply, promote wound healing and prevent infection or wound erosion. The anatomic study on nasolabial subunit is helpful to reconstruct the nasolabial subunit.
