Purpose:Based on the investigation and statistical analysis of patients with cerebral small vessel disease,the distribution of TCM constitution in patients with cerebral small vessel disease and the correlation between brain MRI diagnosis,location,severity of cerebral small vessel disease and TCM constitution types were discussed.In order to find the susceptible constitution of apoplexy and provide reference for the early prevention of apoplexy and clinical individualized physical conditioning.Methods:During the period from January 2018 to January 2019,426 outpatients and inpatients with cerebral small vessel disease were diagnosed by cranial magnetic resonance imaging(MRI)in our hospital.They were willing to cooperate in completing the four diagnoses of traditional Chinese medicine and questionnaire survey.By collecting their imaging and constitution type data,SPASS 20.0 was used to analyze the data,and to explore the distribution characteristics of TCM constitution type of patients with cerebral small vessel disease and the correlation between the location,severity of cerebral small vessel disease and constitution type of TCM.Result:Of 426 patients with cerebral small vessel disease,270 patients had lacunar cerebral infarction,181 patients had leukoaraiosis and 4 patients had cerebral microbleeds.Among them,153 patients had leukoaraiosis with old cerebral infarction,28 patients had lacunar cerebral infarction with leukoaraiosis,and 1 patient had leukoaraiosis with cerebral microbleeds.242 patients had lacunar cerebral infarction alone.The TCM constitution types of patients with cerebral small vessel disease were mainly blood stasis,phlegm-dampness,Yin deficiency and Qi deficiency,accounting for39.2%,30.04%,11.97%and 9.38%respectively.The second was Qiyu(5.69%),Yang deficiency(2.58%),damp-heat(2.34%)and calm(1.40%).In statistics of distribution of various types of cerebral small vessel disease and constitution types of TCM(cerebral microbleeds and special constitution,too few samples of specific qualities,not included in statistical analysis),by X2 test,X2 = 17.587,P<0.05,there were significant differences in different types of constitutional types of cerebral small vessel disease.There were significant differences in severity grade of lacunar infarction among different constitutional types(X2=22.15,P<0.05).There were significant differences in the distribution of blood stasis in lacunar infarction,leukoaraiosis and imaging grade(P<0.05).Conclusion:(1)There are significant differences in the distribution of TCM constitution types in patients with cerebrovascular disease.The number of patients with blood stasis is significantly more than other constitution types.Of the 426 patients,blood stasis is the main constitutional type,followed by phlegm-dampness,yin-deficiency,qi-deficiency,qi-depression,yang-deficiency,damp-heat,calm,no special constitution.(2)There was no significant difference in the location of lacunar infarction among different constitutional types.There was no significant correlation between TCM constitution type and the location of the lesion of lumen infarction.(3)There were significant differences in severity classification of lacunar infarction and leukoaraiosis(P<0.05),and the proportion of blood stasis was the most in grade 3 of leukoaraiosis.Targeted regulation of TCM physique types may be of great significance in the prevention of CSVD.(4)The severity of lacunar infarction and leukoaraiosis was positively correlated with age.The number and severity of lacunar infarction and leukoaraiosis increased with age.
