A Clinical Study On Low-impedance Ideation Therapy Combined With Xiaoyao San In The Treatment Of Mild Depression With Liver Depression And Spleen Deficiency

ObjectionTo observe the clinical efficacy of Thought Imprint Psychotherapy(TIP)combined with Xiaoyao Powder in the treatment of mild depression with liver-qi stagnation and the spleen deficiency,so as to provide new ideas for the clinical treatment of depression and theoretical basis for the clinical application of TIP.Methods1.65 cases were randomly divided into escitalopram group,Xiaoyao Powder group and Xiaoyao Powder psychology group.22 patients in the escitalopram group were treated with escitalopram,22 patients in the Xiaoyao Powder group were treated with Xiaoyao Powder and in accordance with the symptoms of patients with a flexible addition and subtraction,and 21 patients in the TIP plus Xiaoyao Powder group were treated with TIP combined with Xiaoyao Powder.All three treatments lasted for 6 weeks.2.The Hamilton depression scale(HAMD)score and score reduction rate before treatment and after treatment for 2,4 and 6 weeks in the three groups were observed respectively,and the influence,the rate of the three treatment methods on patients' depression mood were evaluated as the main indicators.3.Hamilton anxiety scale(HAMA)score was used to evaluate the effects of three treatment methods on patients' anxiety.4.The effect of the three treatment methods on the TCM syndromes of liver stagnation and spleen deficiency was evaluated by using the scale score of TCM syndromes of liver stagnation and spleen deficiency.5.The adverse reactions in the treatment process and changes of experimental indexes before and after treatment were observed for evaluating the safety of the treatment.Results1.65 cases were included in this study,with 3 cases of shedding(2 cases in the escitalopram group and 1 case in the Xiaoyao Powder group),and 62 cases were completed,with a shedding rate of 4.6%.2.Compared with the escitalopram group and the Xiaoyao Powder group,the TIP plus Xiaoyao Powder group has a better effect on relieving the depressive mood of patients with mild depression of liver depression and spleen deficiency.HAMD scores of patients in the three groups after 2 weeks of treatment were significantly lower than those before treatment(P<0.01),and HAMD scores of patients in the TIP plus Xiaoyao Powder group were significantly lower than those in the escitalopram group and the Xiaoyao Powder group after 2 or 4 or 6weeks of treatment(P<0.01,P<0.05).After 6 weeks of treatment,the effective rate of escitalopram group was 90.0%,and the rates of "cured" and "remarkably improved" were 55.0%.The effective rate of Xiaoyao Powder group was 95.2%and the rates of "cured" and "remarkably improved" were 72.4%.The effective rate of TIP plus Xiaoyao Powder group was 95.2%,and the rates of "cured" and"remarkably improved" were 85.7%.There was no significant difference among the three groups(P>0.05).The rates of "cured" and "remarkably improved" of TIP plus Xiaoyao Powder group was better than that of escitalopram group(P<0.05).3.The alleviation of anxiety in patients with mild depression with liver depression and spleen deficiency in the TIP plus Xiaoyao Powder group was better than that in the escitalopram group and the Xiaoyao Powder group.Before treatment,there was no significant difference in HAMA scores among the three groups(P>0.05);after 2 weeks of treatment,the HAMA scores of the three groups were significantly lower than those before treatment(P<0.01);and HAMA scores of the Chinese medicine psychological group were lower than those of the escitalopram group and the Xiaoyao Powder group(P<0.01,P<0.05).4.The TIP plus Xiaoyao Powder group was superior to the escitalopram group and Xiaoyao Powder group in improving the TCM syndromes of liver depression and spleen deficiency.Before treatment,there was no significant difference in TCM syndrome score of liver depression and spleen deficiency among the three groups(P>0.05).after 2 weeks of treatment,the TCM syndrome score of liver depression and spleen deficiency in the three groups was significantly lower than that before treatment(P<0.01).The TCM syndrome score of liver depression and spleen deficiency in the Xiaoyao Powder group was lower than that in the escitalopram group(P<0.05)after 2 or 4 or 6 weeks of treatment,and the TIP plus Xiaoyao Powder group was lower than that in the escitalopram group and Xiaoyao Powder group(P<0.01,P<0.05).5.Safety evaluation:During the treatment of escitalopram,2 patients were fidgety,1 patient discontinued the drug because of dysphoria,1 patient had symptoms of nausea,and 2 patients had inappetence.After 2 weeks of treatment with escitalopram,the patient was resistant to adverse reactions and no special treatment was given.There was no adverse reaction between Xiaoyao Powder group and Xiaoyao Powder psychology group.the safety tests of the three groups were within the normal range during the clinical study.ConclusionTIP combined with Xiaoyao Powder can effectively alleviate the depression and anxiety of patients with mild depression and liver stagnation and spleen deficiency.The curative effect is better than Xiaoyao Powder and escitalopram;TIP technology can improve the efficient of Xiaoyao Powder.Depressive effect,works faster,no adverse reactions,good safety,worthy of clinical recommendation.
